摘 要:目的调查并比较晚期肿瘤患者及家属对生命支持治疗"预先委托"的态度,为探讨其在中国内地实施的可行性提供一定的依据。方法采用描述性研究设计,调查了上海市某三级甲等肿瘤医院晚期肿瘤患者75例和晚期肿瘤患者家属112名,研究工具包括一般资料调查表及"预先委托"态度问卷。结果患者与家属的态度差异具有统计学意义,但都呈现较为积极的接纳态度;文化程度及宗教信仰对患者的态度存在显著性影响;患者与家属对患者终末治疗选择表现出矛盾的心理,医护人员应给予正确引导。结论晚期肿瘤患者和家属对"预先委托"的接受度较高,具体实施还需医护人员的支持和正确引导。Objective To investigate the preference for advance directives about life sustaining therapy among advanced cancer patients and their families,and offer evidence for the establishment of related medical policy. Methods A total of 75 patients with advanced cancer and 112 relatives of cancer patients were recruited from a 3A cancer hospital in Shanghai. They were investigated with a questionnaire about the attitudes toward advance directives. Results The attitudes toward advance directives between patients and families were significantly different. Anyway,advance directive was highly accepted by both two groups. The influence factors for patients were educational level and religious belief. The patients and families showed ambivalence in the choice of terminal therapy. Conclusion Most of advanced cancer patients and their families can accept advance directives which can be implemented with proper guidance and support from medical staff.
