精神科分级护理量化标准的探讨

摘　　要：目的探讨3种常见精神疾病患者住院期间分级护理的量化标准。方法简明精神病量表（N-BPRS），病情严重程度量表（sI）及躁狂量表（BRMS）、汉密尔顿抑郁量表（HAMD）等分别在108例精神分裂症患者、躁狂症和抑郁症患者入院时及入院后进行测评，比较客观地对3种常见精神疾病患者进行分级护理的量化测评。结果3种精神疾病患者N-BPRS量化分筐如下。特级护理：精神分裂症（80．66±11．22）分；躁狂症（80．05±15．44）分；抑郁症（76．88±9．82）分。一级护理：精神分裂症（67．47±10．18）分，躁狂症（6291±10．19）分，抑郁症（56．63±9．52）分；二级护理：精神分裂症（44．04±8．67）分，躁狂症（3957±9．47）分，抑郁症（37．73±7．75）分。三级护理：精神分裂症（27．97±2．19）分，躁狂症（27．89±2．28）分，抑郁症（27．45±1．31）分。每种疾病各护理级别之间量化分值比较差异有统计学意义。其他相关量表分值支持N-BPRS量表表达分值变化。结论使用N-BPRS量表的评定分值来确定3种常见精神疾病分级护理量化标准是比较可行的选择，且高年资护士经过一定的培训后均能熟练掌握操作流程。Objective To discuss the quantitative criteria of grading nursing of three common psychiatric diseases in psychiatric department. Methods Using SI,N-BPRS, BRMS and HAMD scales to evaluate 180 patients with schizophrenia, mania and depression upon admission and after admission. The quantitative e- valuation of grading nursing of these three diseases was compared. Results For the three common mental diseases N- BPRS scale scores were as follows. Superfine nursing： schizophrenia （80.66±11.22）points;mania （ 80.05± 15.44 ）paints; depression （76.88±9.82）points. Primary care： schizophrenia （ 67.47±10.18 ）points; mania （ 62.91 ± 10.19 ）points; depression （ 56.63 ±9.52 ）points; Secondary nursing ： schizophrenia （44.04±8.67）points; mania （ 39.57±9.47 ）points; depression（ 37.73±7.75 ）points. The. three- level nursing ： schizophrenia（ 27.97±2.19 ） points;mania（27.89±2.28）points;depression（27.45±1.31 ）points. Conclusions Using evaluating scores of N- BPRS scale to determine the quantitative criteria of grading nursing of three common psychiatric diseases is a feasible choice, after training, nurses of high qualification can grasp skillfully the operation process.
