摘　　要：目的:探讨皮肌炎(DM)并发恶性肿瘤的临床特点及实验室指标的变化。方法:对216例DM患者的临床表现及实验室指标进行回顾性分析。结果:DM患者恶性肿瘤的检出率为11.52%,且男性(22.22%)多于女性(7.43%)(P<0.01),并发肿瘤的DM患者平均发病年龄[(59.20±12.47)岁]大于未检出肿瘤患者[(47.20±14.40)岁]。年龄>40岁DM患者的恶性肿瘤检出率高。以DM为首发最终确诊为恶性肿瘤的患者中,DM病程<1年的患者占94.44%。并发恶性肿瘤的DM患者皮肤异色样改变、吞咽困难和血沉增快等症状均高于未检出肿瘤患者。在并发恶性肿瘤的DM患者中,肿瘤先行发生者为16%、同时发生者占72%、后续发生者12%。结论:年龄>40岁的男性DM患者,尤其伴有皮肤异色样改变、吞咽困难和血沉增快者易并发恶性肿瘤。Objective： To investigate the clinical characteristics and the change of laboratory data on the patients who were diagnosed as dermatomyositis （DM）. Methods： The clinical manifestations and the laboratory data on 216 patients with DM were retrospectively analyzed. Results： There were 11.52% DM patients complicated with malignancies. Within them 22.22% of the male patients were complicated with malignancies, which were more than those in the females （7.43%）. The patients with DM complicated with malignancies were older （59.20±12.47 years）than those without malignancies （47.20±14,40 years）, and the patients aged over 40 years had a higher detection rate of malignancy complication. Within the patients with malignancies, 16% of their malignancies were discovered before the DM, 12% were found after DM, and the others （72%） were diagnosed at the same time. The interval time between the onset of DM and malignancies was less than one year in the 94.44% patients with DM at first. Poikiloderma, hard swallow and increased ESR were more frequent in the patients complicated with malignancies than those without malignancies. Conclusion： These results demonstrate that DM patients complicated with malignancies are likely to occur in the elderly males with poikiloderma, dysphagia and increased ESR.
