30F通道与18F通道经皮肾镜术治疗肾结石疗效比较的非随机同期对照研究

摘　　要：目的:比较30 F通道与18 F通道经皮肾镜术治疗肾结石的临床疗效。方法:对112例肾结石患者分别行不同口径单通道经皮肾镜碎石取石术(PCNL),其中30 F通道组66例,18F通道组46例,分析比较两组术中出血量、输血率、手术时间、一期结石取净率及并发症等指标。结果:30 F通道组结石面积较18 F通道组大[(429±280)mm2对比(292±160)mm2];30 F通道组与18 F通道组比较,手术时间短[(111±39)min对比(156±49)min],取石效率(单位时间取石率)高[(3.99±0.37)mm2/min对比(1.37±0.28)mm2/min],一期结石取净率高(80%对比61%),两组比较差异具统计学意义(P<0.05)。但30 F组单位时间出血量大,是18 F组的2倍[(0.297±0.03)g/(L.min)对比(0.143±0.01)g/(L.min)],总出血量多[(33±19)g/L对比(22±21)g/L],输血率高(24%对比9%),两组比较差异具统计学意义(P<0.05),但两组单位结石面积出血量相当[(7.69±0.64)g/(L.cm2)对比(7.63±0.59)g/(L.cm2)]。结论:与18 F通道经皮肾镜术比较,30 F通道经皮肾镜术取石效率高,一期结石取净率高,单位时间出血量大,但单位结石面积出血量两组相当。Objective：To compare the outcomes of 30F-tract and 18F-tract PCNL in the treatment of renal stone.Method：One hundred and twelve patients undergone single-tract PCNL（30F-tract,66 cases;18F tract,46 cases） in our clinic were retrospectively studied.The clinical data including blood loss,transfusion rate,operation time,stone-free rate and complications were compared.Results： 30F-tract PCNL caused faster stone removal speed [（3.99±0.37） mm2/min vs（1.37±0.28） mm2/min]and bleeding speed[（0.297±0.03） g/（L·min）vs（0.143±0.01） g/（L·min）],leading to higher transfusion rate（24 ﹪ vs 9 ﹪）,less operation time[（111±39）min vs（156±49）min] and higher stone-free rate（80 % vs 61 %）.Blood loss per square millimeter of stone in this two groups was comparable.Conclusion： 30F-tract PCNL causes faster stone removal speed,faster bleeding speed and higher stone-free rate.Blood loss per square centimeter of stone is comparable with that of 18F-tract PCNL.
