摘  要:目的探讨听觉脑干诱发电位(ABR)V波反应阈应用于听力测定的意义。方法对325例(594只耳)受检者及40例(80只耳)正常人进ABR V波反应阈及纯音听力测定。结果 40例正常人v波反应阈下10~20 dB相当于其客观听力;325例(594只耳)受检者主观听力检测为中度耳聋66只耳(11%),中重度耳聋101只耳(17%),重度耳聋172只耳(29%),极重度耳聋255只耳(43%);客观听力检测为听力正常167只耳(28%),轻度耳聋154只耳(26%),中度耳聋94只耳(16%),中重度耳聋76只耳(13%),重度耳聋55只耳(9%),极重度耳聋48只耳(8%);纯音听力测定与ABRV波反应阈符合158只耳(26.6%),不符合436只耳(73.4%),其中夸大聋269只耳(45.3%),伪聋167只耳(28.1%)。结论 ABR V波反应阈与纯音听力测定相结合,能估测出较为客观真实的听力,对识别夸大聋及伪聋具有重要意义。Objective To investigate the significance of auditory brainstem response(ABR) V wave applied to audiometry.Methods Totally 594 ears of 325 subjects and 80 ears of 40 normal persons was conducted ABR V response threshold and pure tone audiometry.Results ABR V response subthreshold 10-20dB of 40 normal persons is equivalent to objective hearing.As to subjective hearing test,there are 66 ears(11%)with moderate hearing loss,110 ears(17%)with moderately severe deaf,172 ears(29%)with severe deaf,255 ears(43%)with life-threating deaf in 594 ears(325 subjects);as to objective hearing test,there are 167ears(28%)with normal hearing,154 ears(26%)with mild deaf,94ears(16%)with moderate deaf,76ears(13%)with moderately severe deaf,55ears(9%)with severe deaf,48ears(8%)with life-threating deaf;158 ears(26.6%)of pure tone audiometry are accordance with ABR V response threshold,436 ears(73.4%)are not,including 269 ears(45.3%)of exaggerated deaf and 167 ears(28.1%)of malingering deafness.Conclusion ABR V wave response threshold combined with pure tone audiometry can estimate the actual hearing more objectively,and with significance in identification of the exaggerated deaf and malingering deafness.
