纤维支气管镜及三维重建胸部CT在不典型支气管异物中的应用

摘　　要：目的提高不典型支气管异物的临床诊治水平。方法回顾性分析该院2007～2010年收治的有异物吸入史不典型的支气管异物患者12例临床资料。结果异物种类各异,主要以植物性异物为主(60%),胸部影像学无特异表现;骨性和金属异物可通过肺部三维CT提供异物的精确阻塞部位;所有病例均经纤维支气管镜直视下窥见异物,并通过各种活检钳、鄂口钳等取出异物,成功率100%。结论纤维支气管镜对不典型支气管异物的诊疗价值甚高,但同样详细询问病史及辅助检查诸如三维胸部CT重建亦至关重要。Objective To explore the value of fiberoptic bronchoscopy in diagnosis and treatment of untypical bronchial foreign bodies.Methods The clinical date of twelve patients with untypical bronchial foreign bodies performed fiberoptic bronchoscopy were retrospectively analyzed in our hospital from 2007 to 2009.Results The foreign bodies were varied and mainly were plants （60%）,and the usual chest tomography showed atypical.The obstruction site due to bony foreign bodies and metalic foreign bodies could be exactly scanned by three-dimensional reconstructed images of chest CT.All the bronchial foreign bodies were seen under fiberoptic bronchoscopy and taken away from the bronchi by biopsy forceps and alligator forceps,and the success rate was 100%.Conclusion Fiberoptic bronchoscopy is important in the diagnosis and treatment of untypical bronchial foreign bodies,and the detailed history and accessory examination such as scanning with three-dimensional reconstructed images of chest CT are also important.
