喉罩在无痛纤维支气管镜检查中的应用

摘　　要：目的研究喉罩在无痛纤维支气管镜检查中的应用。方法选择年龄20～71岁,ASAⅠ或Ⅱ级患者,重度肥胖及张口困难者排除在外,行纤维支气管镜检查的气管、支气管疾病患者80例,随机分两组,每组40例。Ⅰ组喉罩用于无痛纤维支气管镜检查,Ⅱ组为对照组(无痛纤维支气管镜检查)。观察各组在麻醉检查期间MAP、HR、SPO2、RR变化,纤维支气管镜置入难易度,检查时间,并发症发生情况。结果两组MAP、HR、SPO2、RR与麻醉前比均有所下降(P<0.05),但都在正常范围,组间比较差异无显著性。检查期间两组SPO2与检查前比均有所下降,Ⅱ组比Ⅰ组下降更多(P<0.05),Ⅰ组纤维支气管镜置入比Ⅱ组容易,检查时间比Ⅱ组短(P<0.01),Ⅱ组有呛咳、鼻衄和憋气发生。结论喉罩用于无痛纤维支气管检查提高检查的安全性及检查效率,减少并发症的发生。Objective To evaluate the safety and feasibility of laryngeal mask airway for painless fiberoptic bronchoscopy.Methods Eighty ASA Ⅰ or Ⅱpatients aged 20～71 years underwent painless fiberoptic bronchoscopy were randomly divided into two groups （forty patients each group）：group I were used laryngeal masks for painless fiberoptic bronchoscopy,group Ⅱcontrol group （no Laryngeal masks）.The variation of mean arterial pressure （MAP）,heart rate （HR）,pulse oxygen saturation （SPO2）,and respiratory rate （RR） during anesthesia and scopy were recorded,and the difficult extend of fiberoptic bronchoscope insertion,scopy duration,and complications were recorded too.Results MAP,HR,SPO2,RR of two groups significantly fell after anesthesia （P 0.05）,but were still in normal range,and there were no significant differences between two groups.Compared with that before scopy,SPO2 of two groups during scopy fell,but SPO2 of group Ⅱ were significantly lower than that of group I （P 0.05）.In group I,fiberoptic bronchoscope insertion were easier than group Ⅱ,scopy duration of group I was shorter than that of group Ⅱ （P 0.01）,and many complications such as bucking,epistaxis and breath holding occurred in group Ⅱ.Conclusion Laryngeal masks using for painless fiberoptic bronchoscopy can enhance the safety and efficiency of fiberoptic bronchoscopy,and decrease complications.
