两种治疗方法对兔膝骨关节炎软骨MMP-1的影响

摘　　要：目的:观察超声波及电针治疗对兔膝骨关节炎(KOA)软骨MMP-1表达的影响。方法:40只新西兰大耳兔随机分为正常组、模型组、超声波组和电针组。除正常组外,其余各组采用左膝伸膝位管型石膏固定6周制作KOA模型。模型成功后,超声波组实施3MHz脉冲式超声波治疗;电针组取梁丘、足三里(后三里)、血海、阳陵泉等穴,疏密波刺激,均每日1次。连续治疗14d后4组大耳兔均处死,光镜下观察左膝股骨内侧髁软骨及基质的HE染色改变,采用Mankin’s评分;免疫组织化学MMP-1测定。结果:与正常组比较,模型组、超声波组和电针组软骨及基质Mankin’s评分均明显升高(0.10±0.32与9.90±1.73、4.81±1.23、5.03±1.20,P<0.01),MMP-1表达也明显升高(13.89±3.10与54.65±9.27、31.64±6.69、34.87±4.23,P<0.01)。与模型组比较,超声波组和电针组两项指标均明显降低(P<0.01)。超声波组与电针组两项指标比较差异无统计学意义。结论:超声波和电针治疗均能降低KOA兔软骨中MMP-1的表达,达到治疗KOA的作用。Objective： To observe the effect of ultrasound therapy vs electroacupuncture on the expression of MMP-1 in the cartilage of rabbit knee osteoarthritis model.Methods： Forty New Zealand rabbits were randomly divided into the normal group,the model group,the ultrasound group and the electroacupuncture group.Any group except the normal group was given straight bit left knee plaster casts to create osteoarthritis.The changes in the pathological morphology and the expression of MMP-1 were observed.Results： By Mankin＇s score,the degree of pathological lesion in model group was severer than other groups（P0.01）.The expression levels of MMP-1 in model group were higher than other groups（P0.01）.There was no significant difference between ultrasound group and electroacupuncture group.Conclusions： Both ultrasonic treatment and electroacupuncture can effectively reduce the expression of MMP-1 in the cartilage,and can be used to treat knee knee osteoarthritis.
