小剂量利尿剂联合厄贝沙坦对高血压患者血糖的影响

摘　　要：目的探讨小剂量利尿剂联合厄贝沙坦对高血压患者血糖的影响。方法 120例高血压患者按门诊和入院先后分为厄贝沙坦组和厄贝沙坦联合氢氯噻嗪组各60例,选择同期健康体检者30例作为正常对照组,观察治疗前后血压、空腹血糖(FPG)、空腹胰岛素(FINS)、餐后血糖(FBG)和糖化血红蛋白(HbAlc)水平,计算胰岛素敏感指数(IAI)。结果治疗组治疗前与正常对照组FPG、FINS FBG、HbAlc、IAI和血压差异均有统计学意义(P<0.05),两治疗组治疗前后FPG、FINS FBG、HbAlc、IAI和血压比较差异均有统计学意义(P<0.05),两治疗组间治疗后FPG、FINS、IAI、FBG、HbAlc和血压比较差异均无统计学意义(P>0.05)。结论高血压患者较正常血压者存在着更明显的胰岛素抵抗,厄贝沙坦可明显改善高血压患者胰岛素抵抗,联合小剂量利尿剂并不影响患者血糖。To discuss the effect of small dose diuretics and Irbesartan on the blood glucose of hypertension patients.120 patients were randomly assigned to Group A（60 cases）treated with Irbesartan,Group B（60 cases）treated with Irbesartan combined with Hydrochlorothiazide,and the control group with 30 normal people.Blood pressure,fasting plasma glucose（FPG）and fasting insulin（Fins）were observed before and after treatment,and the insulin sensitivity index（IAI）calculated.Before treatment,there were significant differences between treatment groups and the control group in Fins,IAI and blood pressure（P 0.05）.Differences in Fins,IAI and blood pressure in treatment groups were significant before and after treatment（P 0.05）.After treatment,there were no significant differences between Group A and Group B in Fins,IAI and blood pressure（P 0.05）.Insulin resistance more obviously exists in hypertension patients than in normal people.Irbesartan can effectively improve the insulin resistance.Irbesartan combined with small dose diuretics doesn＇t influence the blood glucose of hypertension patients.
