摘 要:目的观察Zn2+改性羧甲基纤维素(Zn2+-SCMC)对术后腹腔粘连的预防作用。方法将90只大鼠随机分为A、B、C3组,每组各30只。先行均制作肠粘连模型,取A组腹腔内不放药物,B、C组于腹腔内损伤部位分别置人3ml、浓度3%的羧甲基纤维素(SCMC)及3ml、浓度3%的Zn2+SCMC,术后10d处死大鼠,观察各组腹腔粘连情况。结果大鼠肠壁组织病理:A组纤维细胞增生明显,胶原纤维排列致密;B组纤维细胞增生较明显,胶原纤维排列略密;C组纤维细胞增生较轻,胶原纤维排列较疏松。术后10d腹腔内粘连情况:C组腹腔内轻微粘连,粘连发生率44.8%,B组中度粘连,粘连发生率80.0%,A组广泛粘连,致密,粘连发生率100.0%。A组Ⅲ~Ⅳ级粘连最多为17只,B组Ⅱ级粘连最多12只,C组0级粘连最多16只,C组粘连分级较B组、A组明显减轻,差异有统计学意义(P〈0.01)。结论Zn2+-SCMC能明显减轻术后腹腔粘连程度,其作用优于SCMC。Objective To investigate the Zn2+modifical sodium carboxymethyl cellulose (Zn2+ SCMC) on the prevention of postoperative abdominal adhesion. Methods Ninety rats were randomly divid- ed into three groups: group A, group B and group C. They were first produced adhesions model. Then group A of drugs hold the abdominal cavity, but group B, group C were injected intra-abdominal at injured site 3% sodium carboxymethyl cellulose (SCMC) 3 ml, and 3%Zn2+ -SCMC 3 ml separately. Each group was sacrificed on and abdominal adhesions were observed. Results Pathological change:group A was sig- nificantly more intestinal fibroblast proliferation, collagen fibers arranged in dense, group B was slight more intestinal fibroblast proliferation, collagen fibers arranged in slight dense;group C were less intestinal fibro- blast proliferation, collagen fibers Arranged in loose. The circumstance of abdominal adhesions after postop- erative 10 days: Group C minor intra-abdominal adhesions, the rate of abdominal adhesions is 44. 8%. Group B with moderate adhesion,the rate of abdominal adhesions is 80.0%. But group A extensive adhe- sions and dense, the rate of abdominal adhesions is 100. 0%. There are at most 17 rats on level m-IVadhe- sions of group A. There are at most 12 rats on level I1 adhesions of group B. And there are at most 16 rats on level 0 adhesions of group C. The adhesions grade of Group C obviously lighten than another two group. There is significant difference in adhesion grade (P 〈 0. 01 ). Conclusion Zn2+-SCMC significantly re- duce postoperative abdominal adhesions, its role is better than the SCMC.
