摘  要:目的观察阻断兔左锁骨下动脉(LSA)、左锁骨下动脉加左颈总动脉(LCCA)后脑组织核因子(NF).KB、低氧诱导因子(HIF)-1α的表达及形态学改变。方法健康成年日本大耳白兔24只,雌雄不限,随机分成3组,即阻断左锁骨下动脉组、阻断左锁骨下动脉加左颈总动脉组以及对照组,每组8只,用开胸丝线结扎法建立动物模型,8h后2,3,5-氯化三苯基四氮唑(TIC)染色观察梗死面积,同时观察脑组织病理学改变,并用免疫组织化学检测脑组织中NF-κB、HIF-1α的表达。结果阻断左锁骨下动脉后未见脑组织损伤改变,NF-κB、HIF-1α低表达;阻断左锁骨下动脉加左颈总动脉后,脑组织出现明显缺血、缺氧改变,NF-κB、HIF-1α高表达,阳性单位结果分别为(43.9538±4.9238)、(25.0838±3.3532),阳性率结果分别为(0.4468±0.0717)%、(0.4643±0.0647)%,与对照组比较,差异有统计学意义(P〈0.05)。结论单纯阻断左锁骨下动脉后对脑组织及肢体功能无明显影响,证实覆盖左锁骨下动脉在临床应用中是切实可行的,而同时覆盖左颈总动脉对脑组织损伤较大,在覆盖左颈总动脉前需行旁路手术,保证脑部血供。Objective To study the expression of nuclear factor (NF) -KB and hypoxia inducible factor (HIF)-1α and histopathological changes of the brain in a rabbit model after blocking the left subclavian artery and the left subclavian artery plus the left common carotid artery. Methods Twenty-four healthy adult Japanese white rabbits, regardless of sex, were randomly divided into three groups ( 8 rabbits per group), blocking the left subclavian artery group, blocking the left subclavian artery plus the left common carotid artery group and control group. An experimental rabbit model was set up by using thoractomy. The cerebral infarction was observed by TrC staining, histopathological changes were examined by hematoxylin and eosin (HE) staining, and the expression of NF-KB and HIF-1α was detected by using immunohisto- chemistry at 8th h after blockage. Results There was no sign of cerebral ischemia injury, and the expres- sion of NF-KB and HIF-1α was lower than that of control group after blocking the left subclavian artery. However, there were obviously histopathological changes for ischemia and hypoxia, the expression levels of NF-KB and HIF-1α were higher than those in control group (P 〈 0. 05 ) after blocking the left subclavian artery plus the left common carotid artery, the results of the positive unit were (43. 9538 ± 4. 9238) and (25.0838 ±3.3532), and the results of the positive rate were (0.4468±0.0717)% and (0.4643±0. 0647) % respectively. Conehmion It has no side effects by blocking the left subclavian artery, howev- er, it has severe damage on brain after blocking the left subclavian artery plus the left common carotid artery. It suggests that it is feasible for only covering the left subclavian artery in the clinic and it is essential for bypass surgery to supply brain blood before covering the left common carotid artery.
