摘  要:目的探讨孔雀绿指示的实时显微形态观察法在结核分枝杆菌(MTB)快速检测中的应用价值。方法将去污染MTB阳性痰标本接种于微孔液体培养板中,37℃培养,第5天每天用倒置显微镜观察微孔培养板内MTB菌落形态以及微孔变色情况,观察到变色微孔里特异的索状生长结构表示有MTB生长。将同一份标本同时接种于罗氏固体培养基。将液体培养法的检测结果与罗氏固体培养法检测结果比较,分析孔雀绿指示的实时显微形态观察法检测MTB的敏感性、特异性、耗时。结果与两种方法所有阳性结果比较,孔雀绿指示的实时显微形态观察法检测MTB的敏感性为93.5%,特异性为100%,耗时9d。罗氏固体培养法敏感性为76%,特异性为100%,耗时24d。阳性孔变色的时间中位数为9.3d。结论孔雀绿指示的实时显微形态观察法检测MTB具有很高的敏感性和特异性,且需时较短、不需特殊仪器设备,可作为MTB快速检测方法。Objective To evaluate the use of Malachite green indicator- microscopic observation assay(MG-MOA) for testing Mycobacterium tuberculosis( MTB) in liquid cultures. Methods The decontaminated sputum is inoculated into 24-well plate with liquid culture and L-J culture slant respectively. Then incubated at 37℃. Plates are examinated under an inverted microscope on day 5. If two or more colonies (〉2cfu) with characteristic microscopic cording appearance are detected or the color change in both wells, the result is positive. The results of L-J and MG-MOA were compared and the sensitivity, specificity and time consuming of MG-MOA are evaluated. Results If the result of two methods is the golden standard, the sensitivity and specificity of MG-MOA for testing MTB is 93.5%, 100%. the mean time is 9 days, which of L-J is 76%,100%,24 days respectively.The mean time of color changing is 9.3 days. Conclusion The MG-MOA is rapid, simple, accurate and inexpensive for testing MTB.
