经腹部下腔静脉及肝区冲击触诊加压法心脏声学造影诊断房间隔缺损

摘　　要：本文通过导管测压法观察了30例先天性心脏病患者腹部下腔静脉及肝区冲击触诊加压前后右房压力变化,并在此基础上对40例经手术证实的房间隔缺损病例进行了“经腹部下腔静脉及肝区冲击触诊加压法心脏声学造影”的前瞻性研究。并与30例其他先天性心脏病进行对照。结果证明加压声学造影法可诱发和增强单纯性房间隔缺损患者少量右向左分流,并具有较高敏感性和特异性,提高了常规右心声学造影房间隔缺损检出率。This article reported the pressure change of right atrium of 30 cases of congenital heart disease at different time (before and after right upper abdominal pressor touch) by means of catheterization. On the basis of this experiment, 40 cases of ASD proved by surgery were selected and studied prospectively by contrast echocardiography during rightyipper abbominal pressor touch. 30 cases of other congenitial heart disease were studied as control, the result showed that small right-to-left shunt can be provoked and increased by right upper abdominal pressor touch in most patients with uncomplicated ASD, Contrast echocardiography during right upper abdominal pressor touch had higher sensitivity and sepcificity and could increase the detection of ASD.
