全麻手术病人置入胃管时间的分析与研究

摘　　要：目的探讨全麻手术病人胃管置入的最佳时机。方法选择在全麻下行胃大部份切除手术的男性病人100例,以病人住院号尾数的奇偶分为两组,A组病人在术前1～2 h内由病房护士进行置入胃管,B组病人在全麻诱导后气管置管前置入胃管,研究两组置胃管前、置胃管时病人血压、心率的改变,计算置管一次性成功率并进行分析、比较。结果 A组病人胃管前、置胃管时病人血压、心率与基础值相比明显增加,而B组几乎无明显影响,A组病人置管一次性成功率74%,B组病人置管一次性成功率可达100%。结论气管置管全麻的病人宜选择在麻醉诱导后喉镜直视下使用置管钳置入胃管,尤其适用不配合安置胃管的病人,特别是安置胃管时咽喉部高度敏感的病人。[ Objective] To study the best opportunity of putting the tube into the general surgery patients. [ Methods ] Choosing 100 men patients in hospital most of whose stomach are excised in general anesthesia, and dividing into two groups with the terminal and odd of the number： A group patients is put into the tube by ward nurses in the preoperative 1-2 hours, and group B after induction of general trachea catheter tube placement. The patient blood pressure and heart rate is monitored before and when the tubes are putted into the patients. The change of the catheter one-time success rate is calculated, and then compared and analyzed. [Results] Botli of blood pressure and heart rate and basic value of the A group of patients before and when the tubes are putted significantly increased, compared with almost no significant effect of group B, group A catheter one-time success rate is 74%, and patients of group B catheter one-time success rate can amount to 100%. [ Conclusion ] Tracheal catheter patients appropriate choice of general anesthesia after a direct laryngoscopy narcotic induction into the tube used a pipe wrench, especially suitable for the patient who do not cooperate with the tube putting in, and especially for highly sensitive patients when placed the tube in the back of the throat.
