CT及MRI对非典型脑膜瘤的诊断价值

摘　　要：目的本文通过对20例非典型脑膜瘤的CT及MRI的影像表现回顾性分析,探讨其影像的特殊表现。方法 20例有完整资料,经病理证实为非典型脑膜瘤。5例行CT平扫及增强,12例行MRI平扫及增强,3例行CT平扫后再行MRI平扫及增强,3例行MRV,5例行MRA和MRV。结果肿瘤边缘多不规则,CT及MRI平扫分别呈密度或信号均不均匀,增强后呈不均匀强化。MRI检查还可见肿瘤内流空的血管影、脑膜尾征;MRA及MRV检查可清晰的显示肿瘤的血供及静脉窦受侵。结论非典型性脑膜瘤的影像表现不典型,但仍具有脑外肿瘤的影像学特征,MRI检查在肿瘤定位及了解肿瘤与相邻的组织及血管的关系具有很大的作用。Objective To investigate the special imaging performance of atypical meningiomas by retrospectively analysis of the CT and MR imaging findings of 20 patients.Methods We collected 20 cases of atypical meningiomas confirmed by pathology.5 patients accepted CT scan and enhanced CT scan.12 patients accepted MRI scan and enhanced MRI scan.3 patients accepted MRI scan and enhanced MRI scan after CT scan.3 patients accepted MRV.5 patients accepted MRA and MRV.Results The tumor edge was always irregular.The tumor showed uneven density in CT scan and uneven signal in MR scan,and inhomogeneous enhancement in enhanced scan.MRI examination also showed the shadow of vascular flow void and dural tail sign in the tumor.MRA and MRV examination could clearly show the tumor＇s blood supply and venous sinus invasion.Conclusion The image of atypical meningioma was not typical,but it still showed the imaging features of extracranial tumor.MRI plays an important role in the locating of the tumor and understanding the relationship between tumor and the adjacent tissue and blood vessels.
