摘  要:目的探讨二维斑点追踪成像(STI)技术评价不同病理状态下左室整体收缩功能的临床价值。方法扩张性心肌病患者(DCM组)和高血压病左室肥厚患者(LVH组)各20例,另选同期健康志愿者25例(对照组)。应用STI技术测定各组左室整体纵向应变(GLs)、径向应变(GRs)、圆周应变(GCs)及左室扭转角度(LVtw);同时实时三维超声心动图测定左室射血分数(LVEF),分析心肌应变和扭转参数与LVEF之间的关系。结果各检测参数,对照组与DCM组比较差异均有统计学意义(均P<0.01);与LVH组比较差异无统计学意义(P>0.05);DCM组与LVH组比较差异有统计学意义(P<0.01)。对照组GLs、GRs、GCs及LVtw与LVEF相关性较好,r值分别为-0.73、0.49、-0.52及0.60),GLs、GCs为LVEF的独立预测因子,回归系数beta值分别为-0.67、-0.34(P<0.01);DCM组GLs、GRs、GCs及LVtw与LVEF相关性较好,r值分别为-0.78、0.66、-0.74、0.75。GLs、GCs为LVEF的独立预测因子,beta值分别为-0.52、-0.43(P<0.01);LVH组GLs、GRs、GCs及LVtw与LVEF相关性较好,r值分别为-0.75、0.40、-0.65、0.64,GCs、LVtw为LVEF的独立预测因子,beta值分别为-0.42、0.49(P<0.01)。结论 STI技术能全面了解心脏不同病理状态下心功能的变化情况,为评价左室整体运动提供了一个可靠的检测手段。Objective To assess the clinical value of two-dimensional speckle tracking imaging (STI) in evaluating the global systolic function of left ventricle under different pathologic state. Methods Twenty patients with dilated cardiomyopathy (DCM group) , 20 hypertensive patients with left ventricular hypertrophy (LVH group) and 25 healthy subjects (control group) were enrolled in this study. The systolic parameters including longitudinal strain (GLs), radial strain (GRs), circumferential strain (GCs), LV twist (LVtw) were obtained by STI, and left ventricular ejection fraction (LVEF) was acquired by three dimensional echocardiography, the relationship between myocardium strain, torsion index and LVEF were analyzed. Results There was significant difference of each index between control group and DCM group ( allP〈0. 01 ), there was no significant difference of each index between control group and LVH group (P〉0. 05 ), there was significant differece of each index in DCM group and LVH group (P〈0.01). There was good correlation of GLs, GRs, GCs, LVtw and LVEF in control group, r were -0.73, 0. 49, -0. 52 and 0. 60, respectively. GLs and GCs were the independent predictors of LVEF, beta were -0.67and -0. 34(P〈0. 01). There was good correlation of GLs, GRs, GCs, LVtw and LVEF in DCM group, r were -0. 78, 0. 66, -0.74 and 0.75, respectively. GLs and GCs were the independent predictors of LVEF, beta were -0. 52, -0.43 (P〈 0.01 ). There was good correlation of GLs, GRs, GCs, LVtw and LVEF in LVH group, r were -0.75, 0.40, -0. 65 and 0. 64, respectively, LVtw was the independent predictor of LVEF, beta were -0. 42, 0.49 (P〈0.01). Conclusion STI techinique can quantify metergasis of LV global systolic function in different pathologic state and provide precise information for clinical routine practice.
