专人专护对村镇乳腺癌康复期患者生活质量的影响

摘　　要：目的:探讨专人专护对村镇乳腺癌康复期患者生活质量的影响。方法:选择2005年1月～2008年12月9个村镇乳腺癌术后化疗患者120例,随机分为实验组和对照组各60例,实验组对患者进行健康教育、心理咨询、鼓励患者参加沙龙活动、电话随访及家访、为患者配备合理膳食、督促患者定期合理运动等,对照组采用常规护理方法。采用Zung 1997年编制的焦虑自评量表(SAS)、抑郁自评量表(SDS)、癌症患者生活质量核心问卷(QLQ-C30)进行调查,同时测量患者体重及血甘油三酯水平。结果:干预后实验组SAS和SDS评分、体重和血甘油三酯水平较干预前下降,QLQ-C30各项目总体评分改善(除消化系统症状外),差异均有统计学意义(P<0.05);对照组干预前后各项指标比较差异均无统计学意义。结论:专人专护可以提高村镇乳腺癌康复期患者的生活质量。Objective ： To explore the effect of special nursing care by the specialist nurse on the quality of life of breast cancer patients in the rehabilitation period in rural area. Methods ： 120 village and township breast cancer patients who received postoperative chemotherapy from January 2005 to December 2008 were selected and randomly divided into experimental and control groups （ 60 cases in each group ）. The patients in the experimental group were given health education, psychological consultation, telephone follow - up, home visit, diet guidance and they were also encouraged to participate in social activities, regular and reasonable exercises, the routine nursing care was taken in the control group. The Self - rating Anxiety Scale （ SAS） and Self - rating Depression Scale （SDS） compiled by Zung in 1997 and the core questionnaire on the quality of life of cancer patients （QLQ- C30） were used to make an investigation on the patients and the patient~ body weight and triglyceride levels were measured and determined at the same time. Results： The SAS and SDS scores, body weight and triglyceride levels were lower after intervention than those before intervention in the experimental group and each item of QLQ - C30 was overall improved except for the symptoms of digestive system （ P 〈 0.05 ） ; there was no statistically significant difference in the comparison of each item before and after the intervention in the control group. Conclusion ： The special nursing care by the specialist nurse can improve the quality of life of village and township breast cancer patients in the rehabilitation period：
