急性胆源性感染与肠黏膜屏障相关性研究进展

摘　　要：应用计算机检索CNKI期刊全文数据库及PubMed检索系统,以"急性胆源性感染"、"肠黏膜屏障"、"凋亡"为关键词,检索1998年1月—2011年2月有关急性胆源性感染与肠黏膜屏障的文献。纳入标准:(1)急性胆源性感染发病机制的研究;(2)急性胆源性感染的中、西医治疗;(3)肠黏膜屏障与细胞凋亡。根据纳入标准,粗选128篇关于急性胆源性感染与肠黏膜屏障的文献,精选69篇,最后纳入分析45篇。结果显示肠黏膜屏障功能受损与小肠黏膜上皮细胞过度凋亡密切相关,JNK信号通路是影响小肠黏膜上皮细胞凋亡的主要通路。急性胆源性感染可引起肠黏膜屏障功能障碍,细菌和内毒素移位入血,二次激发炎症级联反应,导致SIRS发生而影响预后。"清"、"下"中药在治疗急性胆源性感染中表现出较好的疗效,但其分子学机理尚未阐明。Using a computer to search CNKI Academic Journal and PubMed search system,with ＂acute biliary infection＂,＂intestinal mucosal barrier＂,＂apoptosis＂as key words,retrieved literatures associated with acute biliary infection and intestinal mucosal barrier from January 1998 to February 2011.Inclusion criteria（：1）pathogenesis of acute biliary infection（;2）the Chinese and Western medicine treatment of acute biliary infection（;3）intestinal mucosal barrier and cell apoptosis.According to inclusion criteria,roughing 128 literatures on acute biliary infection and intestinal mucosal barrier,selecting 69 in them,and finally 45 included in the analysis.The results showed that intestinal mucosal barrier dysfunction is closely related to excessive apoptosis of intestinal epithelial cells,JNK signaling pathway is the main channel that affecting the intestinal mucosal epithelial cell apoptosis.Acute biliary infection can cause intestinal mucosal barrier dysfunction,translocation into the blood of bacterial and endotoxin,and secondary stimulate the inflammatory cascade reactions,leading to the occurrence of SIRS and affecting the prognosis.＂Clearing＂and＂purgative＂Chinese herbs showed better efficacy in the treatment of acute biliary infection,but its molecular mechanism has not been elucidated.
