血清胱抑素C在老年高血压患者早期肾损害中的诊断价值

摘　　要：目的探讨血清胱抑素C(Cyst C)在评价老年高血压患者早期肾损害中的诊断价值。方法将老年高血压患者86例根据尿微量白蛋白(mALB)含量分为微量蛋白尿组和无蛋白尿组,选择门诊健康体检者42例作为对照组。分别检测三组人群的血清Cyst C、血肌酐(Scr)、血尿素氮(BUN)、肾小球滤过率(GFR,根据简化MDRD公式计算)等指标,结果进行组间比较和相关性分析。结果三组人群Scr差异无统计学意义,微量蛋白尿组Cyst C高于无蛋白尿组和对照组(P<0.05),GFR低于无蛋白尿组和对照组(P<0.05),老年高血压患者Cyst C、Scr与尿mALB呈正相关(相关系数分别r=0.748,r=0.572,P<0.05)。结论血清Cyst C反映肾小球滤过功能早期损害敏感性优于Scr,是诊断老年高血压患者早期肾损害的敏感指标。Objective To evaluate the value of serum cystatin C for the diagnosis of early renal impairment in elderly hypertension patients.Methods Of 42 healthy controls and elderly 86 essential hypertensive patients,which were divided into two groups with or without microalbuminuria.The data of serum Cystain C,urine microprotein（mAlb）,serum creatinine（Scr） and GFR were obtained and analyzed.Results The level of Scr showed no significant differences of three groups（P0.05）.The level of Cyst C in microalbuminuria group was higher than that in normoalbuminuria group and control group（P0.05）.GFR serum of microalbuminuria group was lower than those in normoalbuminuria group and control group（P0.05）.Cyst C and Scr levels of the elderly hypertensive patients had positive correlation with urinary mALB（P0.05）.The correlation coefficient of Cyst C and urinary mALB（r=0.748） was higher than the correlation coefficient of Scr and urinary mALB（r=0.572）.Conclusion Cyst C should be a sensitive indicator for early renal impairment of elderly hypertension patients.
