摘　　要：目的:探讨有限切开复位联合外固定支架治疗桡骨远端粉碎性骨折的临床疗效。方法:自2004年9月至2010年8月,采用有限切开复位联合外固定支架治疗桡骨远端粉碎性骨折26例。男21例,女5例,平均34岁。其中开放性骨折5例。按AO/ASIF分型:A3型6例,C2型9例,C3型11例。结果:术后随访6～24个月,平均14个月。术后掌倾角恢复为9°～15°(平均12.4°),尺偏角为18°～27°(平均23.5°),桡骨高度恢复良好。骨折愈合时间为3～10个月(平均5个月)。关节功能疗效根据Dienst评分标准:优10例,良14例,可2例,差0例,优良率达92.3%。无针道感染,无针孔骨折,无医源性神经、血管损伤,无伤口感染及骨髓炎等并发症。结论:有限切开复位联合外固定支架是治疗桡骨远端粉碎性骨折的一种较好的方法,其操作简单、固定可靠、疗效满意、并发症少。Objective. To evaluate the clinical effects of limited open reduction combined with external fixation in treatment of comminuted distal radius fractures. Methods： 26 cases with comminuted distal radius fractures were treats with limited open reduction combined with external fixation from September 2004 to August 2010. There were 21 males and 5 females with an average age of 34 years. 5 cases were open frac- tures. According to AO/ASIF classifications, 6 cases were type A3, 9 cases were type C2, 11 cases were type C3. Results： All the cases were followed up from 6 to 24 months （14 months on average）. After the op- erations, the radial inclination angle was reduced to 9~~15^（12. 4^on average）,the palmar tilt angle was re- duced to 18~~27^（23. 5^on average）, the height of radius was recovered fine. The healing time of fractures were 3---10months（5 months on average）o The joint functions were assessed by Dienst criteria, the results suggested that 10 cases were graded as excellent, 14 good, and 2 fair. No severe complications were found. Conclusions： The treatment of comminuted distal radius fractures by limited open reduction combined with external fixation is of easy performance, reliable fixation, satisfactory effect, and low complications.
