摘 要:目的观察舒芬太尼预处理对糖尿病大鼠烫伤后心肌损伤的保护作用。方法糖尿病SD大鼠40只,随机均分为烫伤组(DB组)、舒芬太尼预处理组(SUF组)、纳洛酮拮抗组(NAL组)和假烫伤组(DS组)。制作30%体表面积Ⅲ度烫伤模型,取烫伤后6h腹主动脉血,测血浆超氧化物歧化酶(SOD)活性、丙二醛(MDA)含量、心肌肌钙蛋白I(cTnI)含量,观察心肌组织形态学改变。结果烫伤后6h,与DS组比较,DB组、SUF组和NAL组血浆MDA和cTnI的含量显著升高(P<0.01),SOD活性显著降低(P<0.01);与SUF组比较,DB组、NAL组血浆MDA和cTnI的含量显著升高(P<0.01),SOD活性显著降低(P<0.01)。结论舒芬太尼预处理糖尿病大鼠可能通过抑制烫伤后氧化损伤等减轻烫伤大鼠过度的伤害性应激反应所导致的心肌损害。Objective This study was designed to investigate the effect of sulfentanyl on burn- induced myoeardiac injury in severely burned diabetic rats. Methods Forty Sprague-Dawley rats were divided into four groups(10 rats per group), including burned group(group DB, diabetic rats given third degree burns over 30% of total body surface area(TBSA) and lactated Ringer's solution for resuscitation), sulfentanyl group (group SUF, diabetic rats given sulfentanyl before burned and lactated Ringer's solution for resuscitation), naloxone group(group NAL, diabetic rats given naloxone before sulfentanyl group), and sham group(group DS, diabetic rats who underwent sham burn). Myocardial histology and serum levels of Superoxide dismutase (SOD), malondialdehyde (MDA), and cTnI were assessed 6 hours later. Results The levels of MDA and cTnI in group DS were lower than that in group DB, SUF and NAL(P〈0.01), but the activity of SOD in group DS was higher than that in group DB,SUF and NAL(P〈0. 01). The levels of MDA and cTnI in group SUF were lower than those in group DB and NAL. Conclusion Sulfentanyl pretreatment has a significant protective effect on burn-induced myocardial injury via inhibiting lipid peroxidation.
