PFNA-Ⅱ及InterTan系统治疗股骨粗隆间骨折疗效评价

摘　　要：目的评价防旋型股骨近端髓内钉-Ⅱ(proximal femoral nail antirotation-Ⅱ,PFNA-Ⅱ)及InterTan系统治疗股骨粗隆间骨折的疗效。方法自2008年6月至2010年5月,在我院手术治疗的55例患者中31例使用PFNA-Ⅱ固定,24例使用InterTan系统固定,比较两组平均手术时间、术中出血量、切口长度、骨折愈合时间、内固定失效例数、术后3个月大腿疼痛(VAS法)程度及术后髋关节Harris评分。结果全部病例随访时间6～23个月,平均16.1个月。PFNA-Ⅱ组手术操作时间、术中出血量较InterTan组少(P<0.05),两组切口长度、骨折愈合时间、内固定失效率、术后3个月大腿疼痛程度和术后髋关节功能均无明显差异(P>0.05)。结论 PFNA-Ⅱ和InterTan系统设计合理,但又各有优势。PFNA-Ⅱ操作简单,组织损伤更小,InterTan理论上具有更高的稳定性。Objective To observe and evaluate the clinical effect of PFNA-Ⅱ and InterTan system in treatment of the femoral intertrochanteric fracture.Methods From June 2008 to May 2010,55 patients with femoral intertrochanteric fracture were treated in my hospital.31 patients were treated with PFNA-Ⅱ and 24 cases were treated with InterTan system.The datas of the 2 groups were collected for statistical analysis on the following aspects：operative time,blood loss,length of incision,clinical healing time of fractures,number of internal fixation failure,the extent of the pain of thigh（VAS method）,functional outcome（Harris Hip Score）.Results Compared with InterTan group,PFNA-Ⅱ had shorter operative time and less blood loss（P0.05）.As to length of incision,clinical healing time of fracture,the number of fixation failure,the pain of thigh and functional outcome,the differences of the two group were of no statistical significance（P0.05）.Conclusion The design of PFNA-Ⅱ and InterTan system is reasonable and the two kinds of intramedullary nails have their own advantages respectively.The operation of PFNA-Ⅱ is more simple with small tissue damage while the fixation of InterTan is more reliable theoretically.
