加味芩珠凉血方及辨证加减治疗寻常型银屑病(血热证)的临床疗效观察

摘　　要：目的观察加味芩珠凉血方及辨证加减治疗寻常型银屑病(血热证)的临床疗效。方法选取105例寻常型银屑病血热证患者,随机分为加味芩珠凉血方辨证加减治疗组39例、加味芩珠凉血方固定方治疗组36例和复方青黛胶囊治疗组30例,用药前后观察三组银屑病面积和严重程度评分法(PASI)的变化。结果临床研究显示,辨证加减治疗组总有效率为77.8%,愈显率为41.7%。固定方组总有效率为61.29%,愈显率为32.3%。复方青黛组总有效率为46.37%,愈显率为3.3%。辨证加减组总有效率明显优于固定方组和复方青黛组,愈显率与固定方组相当,优于复方青黛组,固定方组总有效率及愈显率均优于复方青黛组,经统计学分析差异有显著性(P<0.05)。结论该研究证实加味芩珠凉血方治疗寻常型银屑病血热证的疗效稳定、安全、有效,在此原则上,辨证加减更能提高疗效,减少副反应。Objective To observe the clinical curative effect of blood - heat syndrome psoriasis vulgaris with Qing Re Liang Xue decoction based on syndrome differentiation. Methods 105 patients with blood - heat syndrome psoriasis vulgaris were randomly divided into three groups. Treatment group adopted plus - minus treatment of Qing Re Liang Xue decoction based on syndrome differentiation, control group one adopts fixed prescription to Qing Re Liang Xue decoction, control group two was given compound indigo capsule. The internationally scores of Psoriasis Area and Severity Index（PASI） were usedto measure the skin lesions. Results The total effective rate of plus -minus group of syndrome differentiated treatment was 77.8% , the total effective rate of control group was 61.29% ,and the total effective rate of compound indigo capsule group was 46.37%. The total effective rate of plus - minus group of syndrome differentiated treatment was obviously superior to that of control group and compound indigo capsule group ; the total effective rate of control group is obviously superior to that of compound indigo capsule group. There had significant difference between the two groups （ P 〈 0.05 ）. Conclusion Through this study, treatment of removing pathogenic heat from blood based on syndrome differentiation is again proved as an effective method to cure blood - heat syndrome psoriasis vulgaris. Based on this principle,plusminus group of syndrome differentiated treatment can improve efficacy and reduce side effects.
