甲状腺功能减退症治疗前后血清心肌酶谱和血脂变化

摘　　要：目的探讨原发性甲状腺功能减退症(甲减)患者使用左旋甲状腺激素替代治疗前后血清心肌酶谱和血脂的变化特点,以提高对甲减的认识。方法收集2008年7月-2011年4月在蚌医二附院诊治的原发性甲状腺机能减退症患者58例(其中门诊患者39例,住院患者19例),分别在使用左旋甲状腺激素治疗前以及治疗后6～10周进行血清心肌酶谱(磷酸肌酸激酶、磷酸肌酸激酶同工酶、天门冬氨酸氨基转移酶、乳酸脱氢酶)、血脂(总胆固醇、甘油三酯)检测。结果左旋甲状腺激素治疗前血清心肌酶谱和血脂均有明显升高,经6～10周替代治疗,随甲状腺功能恢复正常,血清心肌酶谱和血脂水平也显著下降,且差异有统计学意义(P<0.05)。结论原发性甲状腺机能减退症患者存在血清心肌酶谱和血脂的升高,经左旋甲状腺激素替代治疗后,两者均能相应下降。Objective To investigate the serum levels of myocardial enzymes and lipids in patients with primary hypothyroidism before and after levothyroxine replacement therapy. Methods Fifty-eight patients with primary hypothyroidism （including thirty-nine clinic patients and nineteen hospitalized patients） were observed from Jhly 2008 to April 2011. The serum levels of myocardial enzymes [ creatine phosphokinase （ CPK）, CPK-MB, aspartate aminotransferase （ AST）, lactate dehydrogenase （ LDH ） ] and lipids [ total cholesterol（TCh） ,Triglyeerides（TG） ] were detected before and after levothyroxine replacement therapy for six to ten weeks. Results The serum levels of myocardial enzymes and lipids were high before treatment, and were significantly decreased with the recovery of thyroid function after substitutional treatment with levothyroxinc, the difference was statistical significant （P 〈 0.05）. Conclusion The serum levels of myocardial enzymes and lipids are high in patients with primary hypothyroidism,the levothyroxine replacement therapy can decrease the levels.
