中国不孕症疾病经济负担分析

摘　　要：目的估计不孕症的疾病经济负担,为不孕症治疗提供费用方面的研究背景及为相关政策制定提供参考依据。方法以中国2008年不孕症患者为对象,以家庭为基础,利用不孕率;不孕症就诊率;不同治疗方式的使用情况和治疗费用;误工情况和交通费用;以及治疗导致的孕产妇发病情况等,进行治疗方式及疾病经济负担种类的估算。结果 2008年不孕症的疾病负担,平均每对夫妻为21 718元,中国2008年不孕症的总疾病负担,估计为810亿元～2300亿元。其中辅助生育技术的贡献率超过50%,手术治疗贡献率约为30%,药物治疗最低,约17%;直接医疗费用所占份额为92%,直接非医疗费用、间接经济负担和无形经济负担所占份额合计为8%。结论不孕症带来的经济负担比大多数疾病更为沉重,且近50%来自于体外受精相关费用,同时直接医疗费用占疾病经济负担的绝大部分份额。Objective To estimate the disease burden caused by infertility in China and to provide scientific evidence for making intervention measures on infertility and public health plan.Methods Data on prevalence rate of infertility,the rate of seeking medical care,medical expenses,and the loss of working time because of infertility in 2008 were collected.The disease burden caused by infertility was calculated according to treatment method and the type of disease burden.Results The disease burden caused by infertility in China was from 81 billions to 230 billions RMB yuan in 2008.The disease burden for each couple was about 21 718 RMB yuan.More than 50% of the burden came from assisted reproductive technologies.The proportion of direct medical cost was 92% of the total cost.Conclusion The disease burden caused by infertility is higher than the most of diseases for a family.The disease burden caused by infertility in China could not be neglected.The main part of the cost is due to in vitro fertilization.The direct medical cost takes majority of the burden.
