摘 要:目的旨在探讨磁共振灌注加权成像技术(perfusion-weighted imaging,PWI)在肺部疾病诊断中的应用价值。方法所有受检者进行常规序列及PWI图像采集。A研究:将31例受检者分为屏气组、自由呼吸组,对所得图像效果进行评分对比。B研究:根据53例受检者的临床资料进行分组,正常组15例,炎症组19例,肿瘤组19例,采用自由呼吸法心电触发PWI进行图像采集,并将所得PWI数据进行单因素方差分析(ANOVA)。结果 A研究:自由呼吸组肺部灌注图像评分高于屏气组[(2.50±0.39)分vs.(0.87±0.35)分,P<0.05]。B研究:平均通过时间、肺血容量、肺血流量在正常组分别为(726.31±222.44)ms、(139.44±80.14)mL/g、(117.95±71.60)mL/(min.100g),在炎症组分别为(846.90±108.53)ms、(96.02±25.76)mL/g、(67.51±13.22)mL/(min.100g),在肿瘤组分别为(1 023.49±286.29)ms、(342.18±117.67)mL/g、(222.18±107.99)mL/(min.100g),各指标组间差异均有统计学意义(P<0.05)。结论自由呼吸法心电触发PWI能够较准确地反映肺部灌注情况。Objective To select the optimal protocol of perfusion-weighted MRI in lung. Methods All subjects were performed pulmonary MRI examinations using routine series and PWI. Study A: 31 subjects performing pulmonary MRI examinations were divided into two groups with holding breath and free-breath randomly. Study B.. All subjects included 15 normal subjects, 19 patients with pulmonary infection, and 19 patients with tumor. The obtained data of the pulmonary perfusion were analyzed statistically using ANOVA and t test. Results Study A.. PWI with free-breath cardiac triggering and respiration compensation could reflect the perfusion of the lung. Study B: The MTT, PBV and PBF among normal pulmonary parenchyma, pulmonary in{ection and tumor were different significantly. Conclusion Pulmonary PWI could be a useful tool in differentiating lung diseases.
