摘 要:目的了解泌尿生殖道感染疑似患者中解脲支原体(Ureaplasma urealyticum,Uu)和人型支原体(Mycoplasma hominis,Mh)感染及药敏情况,以指导临床合理用药。方法对3059例男性尿道、女性宫颈分泌物采集标本,进行支原体培养及12种抗生素的药敏试验。结果受检者总例数3059例,其中Uu感染1384例(45.2%);Mh感染424例(13.9%);Uu+Mh混合感染361例(11.8%)。1384例Uu对抗生素敏感的药敏结果从高到低的前三位依次为:交沙霉素(97.4%)、强力霉素(96.8%)和美满霉素(94.5%);耐药性从高到低的前三位依次为:螺旋霉素(77.4%)、芦氟沙星(76.5%)和依诺沙星(51.2%)。424例Mh对抗生素敏感的药敏结果从高到低的前三位依次为:交沙霉素(94.1%)、强力霉素(93.4%)和美满霉素(86.6%),耐药性从高到低依次为:螺旋霉素(97.6%)、罗红霉素(92.7%)和克拉霉素(90.3%)。361例Uu+Mh感染时,对抗生素敏感的药敏结果从高到低的前三位依次为:交沙霉素(93.4%)、强力霉素(92.5%)和美满霉素(85.0%)。耐药性从高到低的前三位依次为螺旋霉素(97.8%)、罗红霉素(92.2%)和克拉霉素(90.0%)。结论有实验条件的医疗机构在检查支原体感染时宜做药敏选择用药;无条件的,治疗支原体感染时应把交沙霉素、强力霉素、美满霉素等敏感性较高的药物作为首选。Objective To investigate the status quo of infection of the ureaplasma urealyticum (Uu) and mycoplasma hominis (Mh) and drug sensitivity analysis among suspected cases of genitourinary infections to guide proper administration of drugs. Methods Drug sensitivity of l2 antibiotics was assessed based on the secreta specimens collected from the male urethra and female cervix, and culture of mycoplasma. Results 3,059 cases were examined, among which Uu positive (45.2%) was in 1,384 cases, Mh positive (13.9%)in 424 cases, mixed infection (11.8%) in 361 cases. Drug sensitivity was found in 1384 cases infected with Uu. The severity from high to low was arranged in the following manner: Josamyein (97.4%), Doxycline (96.8%), Minomycin (94.5%), etc. and drug resistance was found in Spiramycin (77.4%), Rufloxacin Hydrochloride (76.5%), Enoxacin (51.2%), etc. Drug sensitivity in 424 cases infected with Mh from high to low was found in Josamycin (94.1%), Doxycline (93.4%), Minomycin (86.6%),etc. and drug resistance was found in Spiramycin (97.6%), Roxithromycin (92.7%), Clarithromycin (90.3%), etc. Drug sensitivity in 361 cases infected with Uu and Mh from high to low was found in Josamycin (93.4%), Doxycline (92.5%), Minomycin (85.0%), ect. and drug resistance was found in Spiramycin (97.8%), Roxithromycin (92.2%), Clarithromycin (90.0%), etc. Conculsion Drug sensitivity test should be performed when mycoplasmic infection is explored. If test is not available, drugs with higher sensitivity such as Josamycin, minocycline and doxycycline should be the first choice in the treatment of mycoplasmic infection.
