摘  要:目的探讨低场MRI在隐匿性踝关节损伤中的应用价值。方法对临床高度怀疑隐匿性踝关节损伤而X线检查骨质未见明显异常的36例外伤患者的MRI表现进行分析。结果 36例均表现为隐匿性骨折,其中一处骨折8例,二处骨折22例,三处及以上骨折6例,表现为斑片状T1WI低或稍低信号或等信号,T2WI高或稍高信号或等信号,STIR均表现为高信号,边界欠清;伴跟腱部分撕裂8例,表现为跟腱增粗,跟腱前缘浅凹消失,其内可见点片状高信号影;内侧副韧带部分撕裂2例,外侧副韧带部分撕裂6例,表现为韧带内出现信号增高或韧带周围软组织肿胀呈T1WI低信号、T2WI高信号改变。结论 MRI能准确诊断隐匿性骨折,并可发现韧带、肌腱损伤,具有较大临床应用价值。Objective To investigate the application values of low field MRI in occult ankle injury. Methods The magnetic resonance imaging (MRI) was analyzed in 36 trauma patients who showed no obvious bone abnormalities in X-ray but clinically were highly suspected occult ankle injury. Results All 36 cases were found to be occult fractures, among whom, 8 cases were one fracture, 22 two fractures and 6 three or more fractures. The MRIs showed patchy low, slightly low or equivalent signals in T1WI, high, slightly high or equivalent signals in T2WI, and high signals, less clear boundary in STIR. 8 cases were also accompanied by torn achilles tendon, which indicated the thickened achilles tendon, disappeared front shallow concave with flake high signals. 2 cases were further accompanied by torn medial collateral ligament and 6 cases by torn lateral collateral ligament, showing high signals in the ligaments or low in TIWI, high signals in T2WI surrounding the swollen soft tissue. Conclusion Low field MRI has significant clinical application values in accurately diagnosing occult fractures, and finding the ligament or tendon injuries.
