32例肾上腺外伤性血肿的CT诊断价值

摘　　要：目的探讨CT在肾上腺外伤性血肿中的诊断价值。方法回顾性分析32例肾上腺外伤性血肿患者的CT征象及临床资料。结果 CT平扫示肾上腺区均匀高密度或混杂密度肿块影,边缘尚清,周围脂肪间隙混浊。增强扫描示血肿无明显的强化。10例经手术探查确诊,另22例行保守治疗,复查CT随访发现肾上腺血肿密度逐渐下降,体积逐渐缩小,部分呈囊性变,肾上腺形态缓慢恢复,肾周间隙逐渐清晰。增强扫描发现肾上腺内外支显示逐渐清晰及血肿边缘呈环行强化。结论 CT检查能很好地显示肾上腺血肿的形态、大小、密度以及与邻近组织的关系,可作为诊断肾上腺外伤性血肿的首选方法。Objective To investigate the value of CT scan in the diagnosis of traumatic hematoma of the adrenal gland. Methods A retrospective analysis of 32 cases of patients CT findings and clinical data was done. Results Adrenal CT scan showed homogeneous high-density or mixed density mass shadow,still clear edge,turbid fat space around the adrenal gland. Enhanced scanning showed no obvious enhancement of hematoma. 10 patients were diagnosed by surgical exploration, and the other 22 cases were chosen conservative treatment. Follow-up CT scan confirmed that the adrenal hematoma density decreased during the follow-up, the volume gradually reduced. Some patients showed cystic degeneration, slowed recovery form adrenal gland, perirenal space gradually clear. Enhanced scan showed gradually clear adrenal limb and ring enhancement of the edge of the hematoma.. Conclusion CT examination of adrenal hematoma can show the shape,size,density and the relationship with the adjacent tissue well, and it can be used as the first choice of diagnostic method for traumatic hematoma.
