P16^(INK4A)在宫颈鳞癌中的表达研究

摘　　要：目的:检测P16^(INK4A)在宫颈病变(包括宫颈上皮内瘤变Ⅰ,宫颈上皮内瘤变Ⅱ,宫颈上皮内瘤变Ⅲ,宫颈上皮鳞状细胞癌)中的表达,探讨P16^(INK4A)在宫颈癌的发生发展中所起的作用。方法:采用免疫组化SP法检测32例宫颈鳞状上皮细胞癌、20例CINⅠ,18例CINⅡ,16例CINⅢ和20例因子宫肌瘤手术切除的正常宫颈组织作为对照组的P16^(INK4A)蛋白的表达。结果:①20例正常宫颈组织无P16^(INK4A)蛋白表达;②宫颈鳞癌组织中P16^(INK4A)阳性表达率(96.88%),明显高于宫颈上皮内瘤变组织(43.18%),P<0.05。CINⅠ中15例阴性(-),4例弱阳性(+),1例中度阳性(++),阳性表达率为25%;CINIⅠ中8例阴性(-),3例弱阳性(+),6例中度阳性(++),1例强阳性(+++),阳性表达率为55.56%;CINⅢ中2例阴性(-),3例弱阳性(+),5例中度阳性(++),6例强阳性(+++),阳性表达率为87.5%;宫颈鳞状细胞癌中1例阴性(-),6例弱阳性(+),8例中度阳性(++),17例强阳性(+++),阳性表达率为96.88%。P16^(INK4A)在正常宫颈、CIN和宫颈癌组织中阳性表达率及表达强度逐渐增高,差异有统计学意义。CINⅢ和宫颈癌组织中P16^(INK4A)阳性表达率及表达强度差异无统计学意义。结论:P16^(INK4A)与宫颈鳞癌的发生发展密切相关。Objective：To investigate the expression of P16^INK4A in CIN and SCC, and its role in regulating arises and evolution of cervical carcinoma. Method： SP immunohistochemistry was used to examine the expression of P16^INK4A in 20 cases of CIN Ⅰ, 18 CIN Ⅱ , 16 CINm, 32 cervical squamous carcinoma,and 20 normal cervicaltissues used as control.Results：P16^INK4A didn＇t express in normal cervical epithelium. The expression rate of P16^INK4A in cervical squamous carcinoma （96.88%） was higher than that in CIN （43.18%）. The expression rate in CIN I was 25%, mainly with weak positive expression （＋）; the expression rate in CIN 11 was 55.56%, mainly with moderately positive expression （＋＋）; the expression rate in CIN m was 875%, mainly with moderately positive expression （＋＋） and strong positive expression （＋＋＋）; the expression rate in cervical squamous cell was 96.88%,mainly with strong positive expression（＋＋＋）.Conclusion： P16^INK4A was closely related to the formation and progress of cervical squamous carcinoma.
