腹腔镜下骶棘韧带固定术治疗老年子宫阴道残断脱垂32例分析

摘　　要：目的:探讨腹腔镜下骶棘韧带固定术治疗老年子宫阴道残断脱垂的临床疗效。方法:选取2008年1月～2011年1月64例老年子宫阴道残断脱垂患者为研究对象,将其随机分为对照组32例和观察组32例,对照组采取经阴道骶棘韧带固定术,观察组采取腹腔镜下骶棘韧带固定术,比较两组患者的手术时间、术中出血量、排气时间、尿管使用时间、住院时间、并发症发生率、患者满意率、QOL评分及主客观治愈率。结果:观察组的手术时间、排气时间、尿管使用时间、住院时间均短于对照组,术中出血量少于对照组,并发症发生率低于对照组,患者满意率高于对照组,QOL评分优于对照组,主观治愈率和客观治愈率均高于对照组(P<0.05)。结论:腹腔镜下骶棘韧带固定术治疗老年患者子宫阴道残断脱垂疗效佳,综合优势明显,显著改善患者的状态。Objective： To explore the clinical efficacy of sacrospinous ligament fixation under laparoscope in treatment of elderly patients with uterovaginal and vault prolapse. Methods： A total of 64 elderly patients with uterovaginal and vault prolapse who were treated in the hospital from January 2008 to January 2011 were selected as research objects, then they were divided into control group and observation group randomly, 32 patients in each group, the patients in control group were treated with transvaginal sacrospinous ligament fixation, while the patients in observation group were treated with sacrospinous ligament fixation under laparoscope. The operation times, the amounts of blood loss during operation, the exhaust times, the using times of urinary catheter, the hospitalization times, the incidences of complications, the satisfaction rates, QOL scores, objective cure rates, and subjective cure rates in the two groups were compared. Results： The operation time, exhaust time, using time of urinary catheter, and hospitalization time in observation group were all shorter than those in control group, the amount of blood loss during operation in observation group was less than that in control group, the incidence of complications in observation group was lower than that in control group, the satisfaction rate in observation group was higher than that in control group, QOL score in observation group was higher than that in control group, the objective cure rate and subjective cure rate in observation group were higher than those in control group （ P〈0.05 ） . Conclusion： The clinical efficacy of sacrospinous ligament fixation under laparoscope in treatment of elderly patients with uterovaginal and vault prolapse is good, and the comprehensive advantages are obvious, the method can improve the situation of quality of life of the patients.
