摘 要:目的探讨内质网应激介导的细胞凋亡在子宫内膜异位症(endometriosis,EM)发病中的作用。方法选取2009年1月至2010年12月沈阳市第五人民医院妇产科32例EM患者的异位和在位子宫内膜组织,选取同期28例因浆膜下子宫肌瘤手术并经病理证实为正常子宫内膜组织为对照组。采用流式细胞仪检测子宫内膜细胞凋亡率,采用RT-PCR法检测子宫内膜内质网分子伴侣葡萄糖调节蛋白-78(glucose-regulated protein78,GRP-78)、葡萄糖调节蛋白-94(glucose-regulated protein94,GRP-94)和凋亡因子Caspase-12、Caspase-3mRNA的表达,采用Western blot法检测子宫内膜GRP78、GRP94和Caspase-12、Caspase-3蛋白的表达。结果 EM患者异位和在位子宫内膜细胞凋亡率均显著低于正常子宫内膜,且异位子宫内膜细胞凋亡率显著低于在位子宫内膜;EM患者异位和在位子宫内膜GRP78、GRP94和Caspase-12、Caspase-3mRNA及蛋白表达水平均显著低于正常子宫内膜,且异位子宫内膜GRP78、GRP94和Caspase-12、Caspase-3mRNA及蛋白表达水平显著低于在位子宫内膜。结论 EM患者子宫内膜细胞凋亡减少,内质网应激介导的细胞凋亡途径下调可能是EM发病的机制之一。Objective To study the expression levels of glucose-regulated protein 78 (GRP-78 i, glucose-regulated pro- tein 94 (GRP-94) and Caspase-12, Caspase-3 in the endometrium of patients with endometriosis (EM), and discuss the effects of endoplasmic reticulum stress-mediated apoptosis on the pathogenesis of endometriosis. Methods Ectopic and eutopic endometrium of 32 patients with EM in Shenyang Fifth People Hospital between Jan. 2009 and Dec. 2010 were ob- tained, and 28 normal endometrium were also used for determinations. Apoptotic rates in the endometrium were deter- mined by flow eytometry. GRP78, GRP94, Caspase-12 and Caspase-3 mRNA expression levels were detected by RT- PCR, and GRP78, GRP94, Caspase-I 2 and Caspase-3 protein expression levels were measured by Western blot. Results Apoptotic rates in the ectopic and eutopic endometrium of patients with EM were significantly higher than that of normal endometrium, and the parameter in ectopic endometrium was also significantly higher than that of eutopic endometrium. GRP78, GRP94, caspase-12 and caspase-3 mRNA expression levels in the ectopic and eutopic endometrium of patients with EM were significantly higher than those of normal endometrium, and these parameters in the ectopic endometrium were also significantly higher than those of eutopic endometrium. Conclusion Apoptosis decreased in the endometrium of patients with EM, and down-regulation of endoplasmic reticulum stress-mediated apoptosis might be one of the mechanisms underlying pathogenesis of endometriosis.
