摘　　要：目的探讨全胚冷冻在多囊卵巢综合征/多囊样卵巢(PCOS/PCO)患者实行体外受精-胚胎移植(IVF-ET)中的临床价值。方法回顾性分析PCOS/PCO患者行超促排卵体外受精206个周期的临床资料,其中新鲜胚胎移植周期157例(A组)、全胚冷冻后移植周期49例(B组),比较两组的临床妊娠结局,并分析与临床妊娠率相关的影响因素。结果 B组的胚胎种植率和临床妊娠率高于A组、流产率低于A组,但差异无统计学意义;A组中有1例发生中度卵巢过度刺激综合征(OHSS)。通过单因素和多因素分析结果显示与临床妊娠密切相关的因素是移植优质胚胎数。结论 PCOS/PCO患者采用新鲜胚胎移植与全胚冷冻后适时移植均能获得类似的妊娠结局,影响其妊娠率的关键因素是移植优质胚胎数;全胚胎冷冻是预防中重度OHSS发生的一种有效措施,且对IVF临床结局无不良影响。Objective To evaluate the clinical significance of total embryos frozen in in vitro fertilization and embryo transplantation（IVF-ET） for patients with the polycystic ovary syndrome/polycystic ovary（PCOS/PCO）.Methods The clinical data of 206 IVF cycles under controlled ovarian hyperstimulation,including 157 fresh embryo transfer cycles（Group A）,and 49 all frozen embryo transfer cycles（Group B）,were retrospective analyzed.The clinical pregnancy outcomes were compared between the two groups.Logistic regression was used to analyze the factors related to the clinical pregnancy rate.Results Lower abortion rate,and higher implantation rate and clinical pregnancy rate were revealed in Group B than those in Group A;however,no significant differences was revealed.One case of moderate OHSS was recorded in Group A.According to univariate and multivariate analysis,the clinical pregnancy was significantly correlated with the number of high-quality embryo transferred.Conclusion PCOS/PCO patients with fresh embryo transfer and total frozen embryo transfer can achieve similar pregnancy outcomes,in which the key factor affecting the pregnancy rate is the number of high-quality embryos transferred.Total embryos frozen technique prevents the occurrence of moderate to severe OHSS,without adverse effects on clinical outcomes.
