摘 要:目的 通过4年随访,研究2型糖尿病患者颈动脉硬化度(QCS)及其增长趋势的性别差异。方法 于2006年采集125例2型糖尿病患者(男46例,女79例)临床及实验室检查资料,并使用超声高分辨回声跟踪技术评估受试者的QCS,记作QCS1。并于2010年再次评估受试者的QCS,记作QCS2,计算QCS增加值dQCS。结果 QCS1和QCS2平均值分别为4.6±1.6和7.5±2.4,女性QCS1为(4.5±1.4),与男性(4.9±1.8)比较,差异无统计学意义(t=1.182,P>0.05),女性QCS2为(7.9±2.6),高于男性(6.9±2.1),差异有统计学意义(t=2.263,P<0.05)。男性和女性dQCS平均值为(2.9±2.4),女性为(3.4±2.4),高于男性的(2.0±2.1),差异有统计学意义(t=3.155,P<0.01)。年龄均是男性和女性QCS的独立危险因素(男:r=0.387,P<0.01;女:r=0.487,P<0.01),而dQCS与年龄无显著性相关。结论 年龄是2型糖尿病患者QCS增长的重要因素,女性患者QCS增长速度显著高于男性,随着年龄增加,男性和女性QCS增长速度均呈减慢趋势。Objective To prospectively estimate the gender difference in quantitative carotid stiffness(QCS) in type 2 diabetes mellitus (T2DM)patients. Methods The QCSs of 125 T2DM patients (male :46, female :79 )were measured using an ultrasonic phase-locked echo-tracking system equipped with a high-resolution first in 2006 ( QCS1 ) and again in 2010 ( QCS2 ). And the baseline clinical and laboratory data were also collected in 2006. The mean difference of QCS was calculated. Results The mean value of QCS1 and QCS2 were 4.6 ± 1.6 and 7.5 ± 2.4 respectively. No statistically significant difference was found between male and female patients in QCS1 (4.9± 1.8,4.5 e 1.4, t = 1. 182, P 〉 0.05 ). However, QCS2 in female was higher than that in male(7.9 ± 2.6,6.9 ± 2.1,t = 2.263,P 〈 0.05). The mean value of dQCS was 2.9 ± 2.4. And dQCS in female was larger than that in male (3.4 ±2.4,2.0 ± 2.1, t = 3. 155, P 〈 0.01 ). Moreover, QCS was positively correlated with age in both genders ( male: r = 0.387, P 〈 0.01 ; female : r = 0.487, P 〈 0. 01 ), while dQCS was not correlated with age. Conclusions Age is the most important and independent risk factor for QCS in T2DM. The mean increase of QCS in female is higher than that in male. But the rates of progression trend to slow down with age increasing in both genders.
