1126例病人血浆D-二聚体检测结果分析及临床应用

摘　　要：目的探讨定量检测血浆D-二聚体(DD)在弥散性血管内凝血(DIC)及静脉栓塞疾病(VTE)诊断中的临床价值。方法使用sysmexCA7000全自动凝血仪采用免疫比浊法对1126例来自不同科室的住院可疑病人和66例健康体检者进行血浆D-二聚体定量检测并统计分析。结果不同科室收治的可疑病人根据其病情D-二聚体水平升高的阳性率不同,其检测值与健康组之间比较均有不同的差异(P<0.05)。ICU组:阳性率81%,检测值(851.8±206.6)ng/ml;普外科组:阳性率62%,检测值(433.6±131.4)ng/ml;呼吸内科:阳性率49%,检测值(279.1±92.3)ng/ml;血液内科:阳性率51%,检测值(341.7±102.5)ng/ml;急诊科:阳性率40%,检测值(258.9±91.7)ng/ml。其检测值与健康正常人相比差异有显著统计学意义(P<0.05)。结论 D-二聚体的检测对于临床评价DIC,VTE疾病具有重要的意义。Objective： To determine quantitative detection of plasma D-Dimer and its clinical significance in DIC,VTE.Methods： D-Dimer of 1126 suspicious patients from different Departments and 66 healthy adults were quantitatively analyzed by immunity-turbidity method with Sysmex CA-7000 full-Automatic Coagulation Analyzer.Results： Different hospital departments received different suspicious patients.According to the patients＇ disease,the positive rate of the rise D-dimer levels was different.There was a difference between the patients＇ test value and the healthy persons＇（P〈0.05）.①Intensive Care Unit： The positive rate was 81%.The test value was（851.8±206.6）ng/ml;② Normal Surgical Department： The positive rate was 62%.The test value was（433.6±131.4）ng/ml;③ Respiratory Diseases Department： The positive rate was 49%.The test value was（279.1±92.3）ng/ml;④ Hematological Department： The positive rate was 51%.The test value was（341.7±102.5ng/ml;⑤ Emergency Department： The positive rate was 40%.The test value was（258.9± 91.7）ng/ml.The difference in D-dimer test value was also significant between the patients and the healthy persons（P〈0.05）.Conclusion：The evaluation of the DIC and VTE disease has important significance in testing the plasma D-dimer level.It is closely related to the clinical medicine,which provides accurate and reliable bases for the clinical evaluation.
