摘　　要：目的探讨应用股骨近端解剖锁定钢板治疗高龄股骨粗隆间粗隆下骨折手术方法及临床疗效。方法总结2006年7月～2009年7月收治的应用股骨近端解剖锁定钢板治疗的46例高龄股骨粗隆间骨折和粗隆下骨折患者的临床资料,其中粗隆间骨折30例,粗隆下骨折16例。术中均采用股骨近端解剖锁定钢板行内固定治疗。结果 46例患者的平均随访时间为9.6个月(3～14月),骨折均Ⅰ期愈合,无1例发生断钉及内固定失效。结论高龄患者合并骨质疏松,应用股骨近端解剖锁定钢板治疗高龄股骨粗隆间粗隆下骨折手术方法是一种安全有效的手术方法。解剖锁定钢板起到内固定支架作用,其固定方式使内置物与骨折端形成了"角稳定复合体结构",力学强度明显提高,使骨折稳定强度加大,提高骨折愈合机会,使患者早期进行功能锻炼,早期下地活动,降低卧床并症。[ Objective ] To investigate the therapeutic efficacy and operative method of use that dissection locking proximal femoral bone plate treat the senile femoral intertrochanteric fracture and subtrochanteric fractures. [ Methods ] Forty-six senile patients suffering from femoral intertrochanteric fracture and subtrochanteric fractures in our hospital from July 2006 to July 2009 were chosen. Femoral intertrochanteric fracture was thirty, subtrochanteric fracture was sixteen. According to AO classification, femoral intertrochanteric fracture （A1 case was 10, A2 case was 16, A2 case was 4）. Subtrochanteric fracture （A case was 4, B case was 8, C case was 4）. All the cases were received dissection locking proximal femoral bone plate fixed. [ Resalts ] All the patients were followed up on an average 6 （3-14） months. All of cases were union; none of them appear lag screw oriental failure. [ Conclusion ] The senile patients usually incorporated with osteoposis, and senile femoral intertrochanteric fracture and subtrochanteric fracture received dissection locking proximal femoral bone plate treatment was effective and safety way. The dissection locking proximal femoral bone plate can compose interlocking fixation system, it make the implants and fracture form of that angular stable frame for internal fixation, then enhance mechanical intensity and then make fracture more stably, and raise the opportunity of union. The patients may do physical training earlier that is beneficial to recover the function of work and life activity
