摘  要:目的:观察特制钛网椎板在犬椎板切除椎管成形术后不同时期对硬脊膜外瘢痕形成及粘连的影响,了解其在预防医源性椎管狭窄中的效果,旨在尝试一种新的预防方法。方法:本实验以犬为实验动物,共12只,行随机分组自身对照实验。对12只犬均行静脉全麻,并6只行分节段行单纯椎板切除与另6只行椎板切除特制钛网椎板成形术。于术后4、8周分别摄X线片、磁共振影像(MRI)以及行病理学检查,比较观察椎板切除特制钛网椎板成形与单纯椎板切除术后瘢痕组织增生及椎管成形情况。结果:所有切口均I期愈合。MRI扫描显示椎管成形良好,使用特制钛网椎板椎管成形组的硬脊膜矢状径无明显变窄。硬脊膜未见骨性压迫。病理学提示使用特制钛网椎板比单纯椎管减压的瘢痕组织明显减少。结论:特制钛网椎板应用于椎板切除椎管成形术后能减轻硬脊膜周围瘢痕粘连,从而减小下腰部手术失败综合征的风险。特制钛网椎板固定确实,不易移位,从而避免对硬脊膜压迫的风险。特制钛网椎板能减轻硬脊膜周围瘢痕组织形成及粘连,在脊柱返修手术中可降低手术操作难度。Aim: To study the affection on the prevention and treatment of epidural adhesion, we use a kind of the special vertebral titanic net in different time, to acquaint the effect of preventing the medical source neuro-canal narrow. In order to find a kind of new prevention method. Methods: A total 12 experi- mental dogs are made stochastic grouping. Six experiment dogs are generally anaesthetized by intravenousinjection, made simple laminectomy and the other 6 dogs fixed the special vertebral titanic net after lami- nectomy. After 4 weeks and 8 weeks, two groups are compared to observe spinal fusion and vertebral canal construction, by x-ray, MRI scanning and histology examination again. Results: All incision obtained I stage healing. MRI imagines show prompt vertebral canal reconstruction well, the sagittal direction of duramater capsule is not obvious narrow after using the special vertebral titanic net. Find no compression a- round dura mater. Pathology revealed the special vertebral titanic net results is better than those of simple laminectomy. Conclusion:The special vertebral titanic net results can effectively alleviate the dura around scar, and reduce the "failed back surgery syndrome"(FBBS) after laminectomy. The special vertebral ti- tanic net fix firmly, shift hardly, and avoid the risk of oppressing dura mater capsule. Aalleviating the for-mation and oppressing of the dura around scar, which can reduce the difficulty in re-operations.
