阿托伐他汀对急性心肌梗死致应激性高血糖的干预作用

摘　　要：目的:观察阿托伐他汀对兔急性心肌梗死血糖代谢的调节作用。方法:将60只日本大耳白兔分为空白组(假手术)、对照组(无药物)、20、40、60、80mg/(kg·d)干预组。建立兔急性心肌梗死模型并监测低密度脂蛋白、血糖、肌钙蛋白变化趋势。结果:行冠脉结扎术5组血糖均显著升高(P<0.05vs.空白组),20、40、60mg/(kg·d)干预组血糖峰值明显较低(P<0.05vs.对照组),80mg/(kg·d)干预组血糖峰值较高(P<0.05vs.对照组),行冠脉结扎术5组肌钙蛋白显著升高(P<0.05vs.空白组),20、40、60mg/(kg·d)干预组肌钙蛋白峰值较低(P<0.05vs.对照组),80mg/(kg·d)干预组肌钙蛋白峰值较高(P<0.05vs.对照组)。结论:急性心肌梗死能引起应激性血糖升高,20、40、60mg/(kg·d)阿托伐他汀对应激性高血糖有抑制作用,80mg/(kg·d)阿托伐他汀加重应激性高血糖。Objective To observe the effects of atorvastatin on the hyperglycemia induced by acute myocardial infarction in rabbits. Methods Sixty rabbits were divided into 6 groups： blank group （administered with placebo and sham-operation）, control group （no drugs used）, and 20, 40, 60, 80 mg/（kg·d） of atorvastatin groups （administered with different doses of atorvastatin）. Myocardial infarction in the later 5 groups were induced by permanent ligation of left circumflex coronary artery. Concentrations of low-density lipoprotein cholesterol （tested before and after operation）, blood glucose （tested 1 hour post-operation）, and troponin-I （tested 1 hour after operation） were analyzed. Results Hyperglycemia and the elevation of troponin-I were detected in groups which received permanent ligation of left circumflex coronary artery （P 0.05 vs. blank group）, the levels of blood glucose and troponin-I in 20, 40, 60 mg/（kg·d） of atorvastatin groups were lower than those in control group （P 0.05）, while those in 80 mg/（kg·d） of atorvastatin group were higher （P 0.05）. Conclusions Hyperglycemia can be induced by acute myocardial infarction. Atorvastatin can down-regulates the blood glucose level after myocardial infarction, such effect seems to be on a dose-dependent manner. But higher dose of atorvastatin [80 mg/（kg·d）] may aggravates the hyperglycemia.
