摘　　要：目的:了解上海市徐汇区中心医院抗菌药物使用情况,以促进抗菌药物临床应用规范化监管。方法:随机抽查该院2008-2010年门诊处方,每月100张,住院病人病历每月30份,其中手术和非手术病历各15份,调查抗菌药物使用率、处方频数、联合用药、用药目的、用药时间等。结果:该院门诊与非手术住院病人抗菌药物使用率符合要求,但手术住院病人预防性用药比例过高。门诊存在无适应证用药现象,部分非手术住院病人用药起点过高,部分手术住院病人药物选择及给药时间不规范。结论:该医院抗菌药物的使用存在一定问题,门诊和住院抗菌药物使用管理应各有侧重。Objective： To investigate the utilization of antibacterials in Central Hospital of Shanghai Xuhui District in order to promote the standard management of clinical antibacterials usage. Methods： One hundred outpatients＇ prescriptions and 30 inpatients＇ medical records including 15 surgery and 15 non-surgery records were randomly selected per month from 2008 to 2010. The usage rate, prescription frequency, drug combination, purpose of medication and medication time of antibacterials were investigated. Results： The usage rate of antibaeterials in outpatients and nowsurgery inpatients met the standand, but the preventive medications of antibaeterials were high in surgery inpatients. Some outpatients received antibacterials without definite indication, some non-surgery inpatients received high starting point antibacterials, and unsuitable drug types and medication time were found in a part of surgery inpatients. Conclusion： There are some problems in utilization of antibaeterials in the hospital. The management of antibacterials utilization in outpatients and inpatients should be focused on differently.
