中学生PPD反应强度与结核发病及预防性治疗标准研究

摘　　要：目的探讨PPD反应强度与结核病发病相关性和预防性治疗对象的结核菌素反应标准。方法对不同PPD反应强度的新入学初高中学生进行3年的结核病发病观察,分析PPD反应强度与发病危险性、发病时间的关系,比较不同反应强度预防性治疗对象发病率的变化。结果 7223名健康学生PPD反应强度0～4、5～9、10～14、15～19和≥20 mm或有皮疹水泡者的年均结核病发病率分别为18.2/10万、51.1/10万、115.5/10万、507.0/10万、和709.2/10万;0～4和5～9 mm头2年发病率很低,第3年有所增高,而15～19和≥20 mm或有皮疹水泡者头2年发病率很高,第3年均下降。结论 PPD反应强度与结核病发病存在明显正相关,特别在头2年危险比更大;PPD反应≥15 mm或有皮疹水泡者的发病机会明显升高,以此标准作为当地中学生预防性治疗对象的结核菌素反应标准是比较合理的。Objective To study PPD Intensity of reaction associated with Pathogenesis of tuberculosis and object standard of Preventive treatment objects′ tuberculin.Method Carrying on tuberculosis the outbreak observation for 3 years,to the different responding strength of middle school students who entering school lately,analyze the relationship between the PPD reaction strength and outbreak risk、time of outbreak、Compare incidence changes among the preventive therapy objects who has different intensity ireaction.Results 7223 healthy student PPDs respond strength 0~4、 5~9、 10~14、 15~19 and the ≥ 20 mm or have the average tuberculosis outbreak of the bubble of skin rash the rate distinguish are 18.2/100,000、 51.1/100,000、 115.5/100,000、 507.0/100,000、with 709.2/100,000;0~4 and 5~9 mm the outbreak lead in the first 2 years very low,have to increase high the 3rd years,but 15~19 and the ≥ 20 mm or have the bubble of skin rash in the first 2 years the outbreak lead very high,all descend the 3rd years.Conclusion PPD intensity reaction and TB are obviously just related,specially at the first 2 years,danger compare larger;PPD responds the ≥15 mm or has the outbreak opportunity of the bubble of skin rash to obviously heat up,taking this as the standard of Preventive treatment objects＇ tuberculin of Preventive treatment.
