摘 要:目的探讨静脉输注异丙酚对糖尿病大鼠颈动脉窦压力感受性反射(CSR)的影响。方法健康清洁级成年雄性Wistar大鼠,体重180~220g,采用腹腔注射链脲佐菌素30mg/kg的方法制备糖尿病模型。取糖尿病模型制备成功的大鼠36只,采用随机数字表法,将其随机分为3组(n=12):对照组(DC组)、低剂量异丙酚组(DP.组)和高剂量异丙酚组(DP,组)。另取非糖尿病大鼠36只,采用随机数字表法,将其随机分为3组(n=12):对照组(NC组)、低剂量异丙酚组(NP1组)和高剂量异丙酚组(NP,组)。经右股静脉输注生理盐水12ml·kg^-1·h^-1(对照组)或异丙酚30mg·kg^-1·h^-1(低剂量异丙酚组)或异丙酚60mg·kg^-1·h^-1(高剂量异丙酚组),30min后测定CSR,记录CSR机能参数:阈压(TP)、饱和压(SP)、平衡压(EP)、压力感受器的工作范围(OR),绘制颈动脉窦压力感受器机能曲线,并确定曲线的最大斜率(Ps)和MAP反射性下降的最大值(RD)。结果与DC组比较,DP,组1]P、sP升高,EP、RD和OR降低,DR组TP、sP升高,EP、RD和Ps降低,NC组r11P、sP、EP和OR降低,RD和PS升高(P〈0.01);与DP1组比较,DP,组TP、sP和OR升高,EP、Ps和RD降低,NP1组TP、sP和OR降低,EP、PS和RD升高(P〈0.01);与DP,组比较,NB组TP、SP和OR降低,EP、PS和RD升高(P〈0.01)。结论静脉输注异丙酚可通过窦外机制抑制糖尿病和非糖尿病大鼠CSR,且对糖尿病大鼠CSR的抑制效应更强。Objective To investigate the effect of intravenous infusion of propofol on carotid sinus barore- flex (CSR)in diabetic rats.Methods Healthy adult male Wistar rats weighing 180-220 g were used in this study. Diabetes mellitus was established by intraperitoneal streptozotocin 30 mg/kg and was confirmed by blood glucose i〉 16.7 mmol/L. Thirty-six rats in which diabetes mellitus model was successfully established were randomly divided into 3 groups (n = 12 each) : control group (group DC), low dose propofol group (group DP1) and high dose propofol group (group DP2 ). Another 36 non-diabetic melhtus rats were also randomly divided into the 3 groups (n = 12 each): control group (group NC),low dose propofol group (group NP, ) and high dose propofol group (group NP2 ). Normal saline 12 ml·kg^-1·h^-1 ( control groups), propofol 30 mg·kg^-1·h^-1 ( low dose propofol groups) or propofol 60 mg·kg^-1·h^-1(high dose propofol groups) was infused iv through right femoral vein, and CSR was determined 30 min later. The intrasinus pressure(ISP)-MAP relation curve was constructed and CSR pa- rameters including threshold pressure (TP), saturation pressure (SP), operation range (OR), peak slope of the curve (PS) and MAP maximum reflex decrease (RD) were recorded. Results TP and SP were higher, EP, RD and OR lower in group DPl , and TP and SP were higher, EP,RD and PS lower in group DP2 , and TP,SP,EP and OR were lower, RD and PS higher in group NC than in group DC ( P 〈 0.01 ). TP, SP and OR were higher, EP, PS and RD lower in group DP2 , and TP,SP and OR were lower, EP,PS and RD higher in group NP1 than in group DP, ( P 〈 0.01 ). TP, SP and OR were lower, EP, PS and RD higher in group NP2 than in group DP2 ( P 〈 0.01 ). Conclusion Intravenous infusion of propofol can inhibit CSR in diabetic or non-diabetic rats through outside carotid sinus mechanism, and the inhibition is stronger in diabetic rats than in non-diabetic rats.
