摘  要:目的评估一种新型枕颈内固定系统——枕骨板障间螺钉-棍-椎弓根螺钉在临床应用中的安全性和有效性。方法对2004年1月—2009年1月收治的12例寰枢椎失稳患者、3例Arnold-chiari畸形患者、2例寰枢区肿瘤患者进行枕骨板障间螺钉-棍-椎弓根螺钉系统内固定。结果所有患者术程顺利,术后无神经缺失症状加重。1例出现脑脊液漏,引流1周后愈合;1例出现伤口感染,经局部换药后愈合。随访6~32个月,平均随访时间19个月。17例患者均存活,3~11个月达到骨性枕颈融合,无断钉、断棍及内固定松动病例。结论枕骨板障间螺钉-棍-椎弓根螺钉固定系统可以为枕颈融合术患者提供可靠的稳定性,安全有效,操作简便。Objective To evaluate clinical safety and efficacy of a new occipitocervical fixation system--diploe screw-rodpedicle screw. Method From January 2004 to January 2009, 12 patients with atlantoaxial instability, 3 patients with Arnold-chiari's malformation and 2 cases of atlantoaxial tumors received occipitocervical fusion by internal fixation with diploe screwrodpedicle screw. Results All patients underwent surgery smoothly. No further neural deficiency occurred during surgery. Cerebrospinal leakage occurred in 1 case and was resolved after one week's drainage. Wound infection in 1 case recovered after local dressing change. All cases were followed up from 6 to 32 months with the average of 19 months. All of 17 patients survived and gained occipitocervical fusion from 3 to 11 months after surgery. No screw loosing or rod breakage, and no fixation failure were found. Conclusion Diploe screw-rod-pedicle screw fixation system is safe, effective and convenient which can orovide reliable stabilitv for the oatients who need occiDitocervical fusion.
