先天性胆总管囊肿外科治疗临床分析

摘　　要：目的探讨先天性胆总管囊肿的治疗方式。方法21例先天性胆总管囊肿患儿，采用囊肿切除、胆囊切除、肝总管空肠Roux-en-Y吻合、人工乳头成形术。结果21例患儿均术后恢复良好，无吻合口瘘、吻合口狭窄等并发症，肝功能恢复正常，治愈出院。结论先天性胆总管囊肿切除，胆道重建和人造乳头抗反流的应用，很好地防止了肠、胰液逆流至肝总管，明显减少了胆道感染发生，是治疗先天性胆总管囊肿的理想术式。Objective To study the treatment of congenital choledochal cyst. Methods Twen- ty-one cases of congenital choledochal cyst were treated with cyst resection, cholecystectomy, liver duct- jejunum Roux-en-Y anastomosis and artificial theleplasty. Results Twenty-one cases recovered well postoperatively. No anastomotic fistula, anastomotic stenosis or other complications. Liver functions re- turned to normal, and all patients were cured and discharged. Conclusions Resection of congenital choledochal cyst, biliary reconstruction and application of artificial nipple anti-reflux can prevent intesti- nal, pancreatic juice counter flow to hepatic duct well, significantly reduce biliary tract infection. It is the ideal operation method for the treatment of congenital choledochal cyst.
