摘  要:目的分析儿童和成人猫抓病在临床表现、实验室检查及淋巴结病理方面的不同特点。方法对57例住院儿童和同期住院的45例成人猫抓病患者的临床资料进行回顾性分析。结果儿童和成人猫抓病均有淋巴结肿大。儿童组皮疹、乏力、头痛、咽痛及神经系统症状的发生率比成人组高,成人组胃肠道症状更多见。儿童组多个(≥2个)淋巴结肿大发生率较成人组高,而成人组单个淋巴结肿大几率高。淋巴结肿大部位:儿童组以颈部、腋窝为主,成人组以腹股沟、腋窝为主。儿童组白细胞总数、血沉升高几率显著高于成人组。两组误诊率无差异。结论儿童猫抓病临床表现更加多样化,发生多个淋巴结肿大的几率高,两组间淋巴结肿大部位及病理分期有明显差异,儿童与成人猫抓病误诊率均较高,应予以重视。【Objective】 To compare the differences in the clinical features,laboratory tests and lymph nodes pathology between children and adults with cat scratch disease(CSD).【Methods】 The retrospective study was performed in 57 children and 45 adults with CSD.【Results】 The children and adults with CSD all have lymphadenectasis.In the experiment,the incidence rate of rash,hypodynamia,headache,pharyngodynia and nervous system symptoms were higher in children with CSD.However adults with CSD had larger incidence rate in gastrointestinal symptoms.The rate of more than one lymphadenectasis of children was higher than that of adults,while the rate for single lymphadenectasis of adults was higher than that of Children.The lymphadenectasis located mainly in neck and armpits for children,but most in groin and armpit for adults with CSD.What is more,the incidence rate of WBC and ESR increased were significantly higher in children than those in adults.There was no difference in misdiagnosis rate for the two experimental groups.【Conclusions】 The clinical features in children with SCD was more diverse.The children are easier to get more than one lymphadenectasis.Great difference-s for lymphadenectasis location and pathological stage were existed in the two experimental groups.Last,high misdiagnosis rate in both groups should be emphasized.
