嵌顿性胆囊结石行腹腔镜手术的疗效分析

摘　　要：目的:探讨嵌顿性胆囊颈部结石及胆囊管内结石施行腹腔镜手术的疗效及方法。方法:回顾分析为256例嵌顿性胆囊颈部结石及胆囊管内结石患者行腹腔镜手术的临床资料。结果:手术时间平均(50±4.5)min,术中出血量平均(60±12)ml,平均住院(4±1.2)d。4例发生并发症,其中,肝总管夹闭1例,胆囊管残端漏1例,术后出血1例,结肠损伤1例。术后随访1～3个月,疗效可靠。结论:嵌顿性胆囊颈部结石及胆囊管内结石施行腹腔镜手术较困难,采取积极有效的措施,避免或减少并发症发生,才能取得良好疗效。Objective：To explore the efficacy and method of laparoscopic operation in treatment of impacted calculi in neck of gallbladder and cystic duct.Methods：The clinical data of 256 cases of impacted calculi in neck of gallbladder and cystic duct who underwent laparoscopic operation were retrospectively analyzed.Results：Complications occurred in 4 cases,common hepatic duct clipping,cystic duct bile leakage,postoperative bleeding and colon injury occurred in 1 case respectively.Conclusions：It is difficult to perform laparoscopic operation in cases of impacted calculi in neck of gallbladder and cystic duct.Active measures should be taken to lessen the complications and to get better results.
