法乐四联症根治术76例治疗体会

摘　　要：目的 总结７６例法乐四联症根本治术情况，介绍如何减少术后并发症的经验。方法 全组均在全麻低温体外循环下手术，包括室间隔缺损修补，右心室流出道拓宽。７１例用涤沦衬心包片；５例用自体心包片拓宽；２４例跨环补片。结果 术后死亡１２例。主要死因为低心排综合征，灌注肺和多脏器功能衰竭。结论 加强围术期处理，畸形纠正满意，心肺保护良好对法乐四联症根治术后并发症的防治至关重要。Objective:This paper was conducted to describe the experience of decreasing the early postoperative complications of total correction of tetralogy of Tallot in 76 cases Methods:All of the cases were totally correted under deep hypothermia with CPB The procedure included VSD repair with dacron patch,valvotomy for pulmonary valve stenosis,resection of the hypertrophic right infundibulum and enlargement of the RVOT and pulmonary trunk There were 24 cases with transannular patching Dacron with autogenous patch was used in 71 cases and autogenous pericardial patch in the other Result:There were 12 postoperative death from LCOS,ARDS and MSOF Conclusion:It is important to improve perioperative management,satisfactory correction of the anomalies,good perioperative protection of the heart and lung so as to minimize the complications
