经皮前路螺钉内固定治疗齿状突骨折临床疗效分析

摘　　要：目的探讨经皮前路螺钉内固定治疗齿状突骨折临床疗效。方法对16例成人新鲜齿状突Ⅱ、Ⅲ型,有寰枢椎脱位者术前先行颅骨牵引复位,X线片显示复位满意后在C形臂X线监视下,行前路加压螺钉内固定治疗,分析其治疗效果及并发症。结果所有患者术后均获得随访,随访时间为6～36个月,平均23.6个月,X线片显示所有患者骨折均达到骨性愈合,无明显颈部运动受限,无螺钉断裂、移位及神经损伤等并发症。结论在严格把握适应证、详细的术前准备、精确的手术操作情况下,经皮前路螺钉内固定治疗齿状突骨折具有微创、内固定牢靠、愈合率高、并发症少等优点,是一种不错的治疗选择。Objective To analysis clinical efficacy of percutaneous anterior cannulated screw fixation for odontoid fractures. Meth- ods 16 cases of adult type Ⅱ, Ⅲ fresh odontoid, atlantoaxial dislocation with preoperative skull traction first,X - ray showed satisfactory reduction in the C - arm X - ray monitoring, anterior compression screw fixation, analysised its treatment and complications. Results All patients were followed up from 6 - 36 months, with an average of 23.6 months. X -ray showed that all patients had reached the bone frac- ture healing, no significant neck movement was limited,no screw breakage,displacement,and nerve injury. Conclusion Under the strictly indications, detailed preoperative preparation, precise surgical operating conditions, pereutaneous anterior odontoid screw fixation of fractures with minimally invasive,internal fixation, healing rate, fewer complications,is a good treatment options.
