西地那非治疗先天性心脏病肺动脉高压的疗效观察

摘　　要：目的肺动脉高压(pulmonary arterial hypertention,PAH)是左向右分流型先天性心脏病(congenital heart disease,CHD)常见且严重的并发症,是影响先心病外科治疗效果的重要因素,本文探讨西地那非治疗CHD合并PAH的临床治疗效果。方法研究对象为2009年8月至2011年3月我院收治的20例心内左向右分流、平均肺动脉平均压>25mmHg的CHD患者,给予西地那非口服,一次20mg,每6小时一次,对比用药前后患者平均肺动脉平均压力的变化。结果西地那非连续使用5d后,患者平均肺动脉压力明显下降。结论在CHD合并PAH的治疗中,西地那非可以特异地降低肺动脉压力,改善心功能,降低死亡率,其使用方便,可为CHD合并PAH治疗的首选。Objective Pulmonary hypertension（PAH） is a fairly common and serious leisure succeed to congenital heart disease（CHD）,it is a important agent to affect surgical therapeutic efficacy.To investigate the clinical efficacy of sildenafil citrate for treatment with PAH secondary to CHD.Methods we randomly assigned 20 patients with intracardiac left-to-right shunt and pulmonary artery mean pressure25 mmHg to sildenafil citrate（20mg） orally one time every 6h.We measure the Pulmonary artery Pressure（PAP） before medication and on operation.Results The PAP on operation were lower than before medieation（P〈0.01）.Conclusions It Prompt that Sildenafil citrate can reduce pressure of pulmonary artery and improve heart function.
