血脂康对急性冠脉综合征患者淋巴细胞亚群的影响

摘　　要：目的观察血脂康对急性冠脉综合征(ACS)患者淋巴细胞亚群及T淋巴细胞活化的影响。方法178例住院确诊为冠心病(CAD)患者,其中稳定型心绞痛(SAP)50例,不稳定型心绞痛(UAP)45例,急性ST段抬高型心肌梗死(STEAMI)43例,急性非ST段抬高型心肌梗死(NSTEAMI)40例,常规治疗基础上加用血脂康,于治疗前及治疗后4周采用流式细胞仪分析淋巴细胞亚群变化及T淋巴细胞活化情况。结果治疗前UAP组、STEAMI组和NSTEAMI组Th细胞百分比及Th/Ts细胞比值高于SAP组(P<0.05),Ts细胞百分比降低(P<0.05),Th细胞激活百分比显著升高(P<0.001);治疗4周后,UAP组、STEAMI组和NSTEAMI组Th/Ts细胞比值显著降低(P<0.01),Th细胞激活百分比降低(P<0.005),与SAP组比较无显著差异(P>0.05)。结论急性冠脉综合征患者存在细胞免疫功能异常,血脂康可能通过调节细胞免疫功能,稳定粥样硬化斑块。Objective To investigate the effect of XueZhiKang on T lymphocyte subgroup and T lymphocyte activation in patients with acute coronary syndrome（ACS）.Methods 178 coronary artery disease patients were chosen,among which,50 with stable angina pectoris（SAP）,45 with unstable angina pectoris（UAP）,43 with ST segment elevate acute myocardial infarction（STEAMI） and 40 with non-ST segment elevate acute myocardial infarction（NSTEAMI）.T lymphocyte subgroup and T lymphocyte activation percentage were detected before and after 4 weeks of XUEZHIKANG treatment.Results Comparing with SAP group,the percentage and the ratio of Th/Ts were higher（P〈0.05 or 0.01）,Ts percentage were lower（P〈0.05） and the activated Th lymphocytes were significantly higher in UAP,STEAMI and NSTEAMI group（P〈0.01）.After 4 weeks of XUEZHIKANG treatment,the ratio of Th/Ts were decreased,the activated Th lymphocytes were decreased in UAP,STEAMI and NSTEAMI group（P〈0.05 or 0.01）.There＇s no differences comparing with SAP group（P〈0.05）Conclusion T lymphocyte activated and the immunomodulation function were disturbed in ACS patients.XUEZHIKANG may modulate immunomodulation function and this may contribute to the stability of the plaque.
