2008～2010年卫生部心肌标志物室间质评结果回顾评价

摘　　要：目的通过对心肌标志物室间质评成绩的分析,查找不足,改进提高检验质量,更好地服务于患者和临床工作。方法将2008～2010年6次参加卫生部室间质评的心肌标志物指标进行列表统计,通过分析当前性能解释和累积性能解释,结合仪器情况和日常工作,巩固成绩,查找不足。结果 2008年第2次心肌标志物蛋白类首次参加卫生部室间质评,肌钙蛋白(TPI)得分为0%使当前性能解释为不满意,直接导致后两次累积性能解释为失败;2009年第2次肌酸磷酸激酶同工酶(CK-MB)得分60%,仅当前性能解释不满意,累积性能解释为成功。对比2008～2009年质评结果,2010年各指标当前性能解释均满意且得分100%,累积性能解释也均成功且得分100%。结论 2008～2010年卫生部心肌标志物蛋白类室间质评从无到有,再发展到具体针对到指定仪器方法学的质评标准,细化了不同检测系统的评价,更好地达到了质量控制的目的。同时,2010年通过加强操作人员培训,强化日常维护保养等改进措施全面提升了仪器的稳定性和准确性,为取得准确的检测结果做出了努力。Objective To summarize the results of quality assessment for myocardial markers over these years,and to explore changes in criteria for external quality assessment,to find drawbacks and to improve eventual quality assessment and to provide better medical service for patients and clinical work.Methods The results of external quality assessment of myocardial markers in 2008 to 2010 were statistically analyzed for interpretation of current and cumulative performances,and also combined with condition of instruments and daily work to consolidate these achievements.Results In 2008,this department involved in external quality assessment of myocardial protein markers for the first time by the Ministry of Health,TPI scored 0% was not satisfied with the current interpretation,a direct result of interpretation for next two cumulative performances as an explanation for failure.In 2009,2nd myocardial marker CK-MB scored 60%,it only got unsatisfied interpretation for the current performance,and it got successful interpretation in cumulative performances.In comparison with results of quality assessment in 2008-2009,the results in 2010 satisfied the interpretation of current performance and scored as 100%,and the interpretation of cumulative performances was successful and scored as 100%.Conclusion The Ministry of Health had established the criteria for external quality assessment of myocardial markers in 2008~2010,and further developed criteria for specific instruments and specific methodology for quality assessment,and detailed evaluation of different detection systems,thus it got better result for meeting the quality control.Meanwhile,this department made effort through strengthening the training of operators,routine maintenance and other improvement to enhance stability and accuracy of instrument in 2010,thus it may obtain result of accurate examinations.
